摘要
目的:探讨小夹板外固定联合中药熏洗对老年C型桡骨远端骨折患者的影响。方法:选取2021年1月—2022年1月漳州市中医院收治的老年C型桡骨远端骨折患者80例,采用掷硬币法随机分为对照组(n=40)和观察组(n=40)。对照组接受小夹板外固定治疗,观察组接受小夹板外固定联合中药熏洗治疗。比较两组临床疗效、炎症因子、氧化应激、血清疼痛介质水平、肿胀指数、疼痛度。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05);观察组治疗后C反应蛋白(CRP)、白细胞介素-1(IL-1)、白细胞介素-6(IL-6)、丙二醛、晚期氧化蛋白产物、P物质(SP)、前列腺素E_(2)(PGE_(2))、β-内啡肽(β-EP)与神经肽(NPY)水平均低于对照组,谷胱甘肽过氧化物酶与超氧化物歧化酶水平均高于对照组,差异均有统计学意义(P<0.05);观察组治疗后3 d、5 d肿胀指数、疼痛数字评分法(NRS)评分低于对照组,差异均有统计学意义(P<0.05)。结论:小夹板外固定联合中药熏洗治疗老年C型桡骨远端骨折患者,可提升治疗效果,降低炎症因子水平,改善氧化应激反应,降低血清介质水平、肿胀指数与疼痛度。
Objective:To investigate the effect of small splint external fixation combined with traditional Chinese medicine fumigation and washing in the treatment of type C distal radius fractures in elderly patients.Method:A total of 80 elderly patients with type C distal radius fractures who admitted to Zhangzhou Traditional Chinese Medicine Hospital from January 2021 to January 2022 were selected,they were randomly divided into the control group(n=40)and the observation group(n=40)according to the coin toss method.The control group was treated with small splint external fixation,the observation group was treated with small splint external fixation combined with traditional Chinese medicine fumigation and washing.The clinical efficacy,inflammatory factors,oxidative stress,serum pain mediators,swelling index and pain degree were compared between two groups.Result:The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).The levels of C-reactive protein(CRP),interleukin-1(IL-1),interleukin-6(IL-6),malondialdehyde,advanced oxidation protein products,substance P(SP),prostaglandin E_(2)(PGE_(2)),β-endorphin(β-EP)and neuropeptides(NPY)of the observation group after treatment were lower than those of the control group,the levels of glutathione peroxidase and superoxide dismutase were higher than those of the control group,the differences were statistically significant(P<0.05).The swelling index and numerical rating scale(NRS)scores of the observation group at 3 d and 5 d after treatment were lower than those of the control group,the differences were statistically significant(P<0.05).Conclusion:Small splint external fixation combined with traditional Chinese medicine fumigation and washing in the treatment of elderly patients with type C distal radius fracture can improve the therapeutic effect,reduce the level of inflammatory factors,improve oxidative stress response,reduce serum mediator level,swelling index and pain degree.
