摘要
目的比较常规超声引导下穿刺活检与超声造影引导下穿刺活检对肝癌鉴别诊断的价值。方法选取82例疑似肝癌患者作为研究对象,所有患者分别进行病理学检查、常规超声引导下穿刺活检及超声造影引导下穿刺活检。以病理学检查结果为“金标准”,比较常规超声引导下穿刺活检与超声造影引导下穿刺活检对肝癌鉴别诊断的价值。结果超声造影引导下穿刺取材成功率100%,明显高于常规超声引导92.20%(P<0.05)。82例疑似肝癌患者病理结果:原发性肝细胞癌24例、毛细胆管癌5例和转移癌9例,脂肪肝23例、酒精性肝病11例、肝硬化6例和肝炎4例。超声造影引导下穿刺活检灵敏度为89.47%、特异性为93.18%、准确度为91.46%,均高于常规超声引导下穿刺活检(68.42%、77.27%、73.17%),差异有统计学意义(P<0.05)。两种引导方式并发症发生率比较,差异无统计学意义(P>0.05)。结论与常规超声引导下穿刺活检比较,超声造影引导下穿刺活检对肝癌具有较高的诊断价值且安全性更高。
Objective To explore the characteristics and value of needle biopsy guided by conventional ultrasound and needle biopsy guided by contrast-enhanced ultrasound in the differential diagnosis of liver cancer.Methods A total of 82 patients with suspected liver cancer accepted by our hospital were selected as the research subjects.All patients underwent pathological examination,conventional ultrasound-guided needle biopsy,and ultrasound-guided needle biopsy.Taking pathological examination results as the"gold standard",compare the value of conventional ultrasound-guided needle biopsy and contrast-enhanced ultrasound-guided needle biopsy in the differential diagnosis of liver cancer.Results The success rate of needle biopsy under the guidance of contrast-enhanced ultrasound was 100%,which was significantly higher than that of conventional ultrasound guidance of 92.20%(P<0.05),the pathological results of 82 patients with suspected liver cancer:including 24 cases of primary hepatocellular carcinoma and 5 cases of cholangiocarcinoma and 9 cases of metastatic cancer,23 cases of fatty liver,11 cases of alcoholic liver disease,6 cases of liver cirrhosis and 4 cases of hepatitis.Contrast-enhanced ultrasound-guided needle biopsy has a sensitivity of 89.47%,a specificity of 93.18%,and an accuracy of 91.46%,which are higher than those of conventional ultrasound-guided needle biopsy(68.42%,77.27%,73.17%),the difference was statistically significant(P<0.05),the complication rate of the two guiding methods was not statistically significant(P>0.05).Conclusion Compared with conventional ultrasound-guided needle biopsy,ultrasound-guided needle biopsy has higher diagnostic value and higher safety for liver cancer.
