摘要
目的探讨耳穴压豆联合睡眠干预在骨科老年患者术后认知障碍(POCD)中的应用效果。方法选取2020年11月—2021年11月在上饶市人民医院骨科行手术治疗的患者共60例,采用随机奇偶数法分组,对照组30例,实施常规护理;观察组30例,实施耳穴压豆联合睡眠干预。对比2组干预前后临床症状评分、POCD的发生率、生活质量及护理满意度。结果术后3 d、7 d,观察组MMSE、ADL均显著高于对照组,PSQI显著低于对照组;观察组术后3 d、7 d POCD发生率均显著低于对照组;观察组生活质量评分、护理满意度均显著高于对照组。结论在骨科老年患者术后认知障碍中应用耳穴压豆联合睡眠干预,可明显改善患者睡眠质量,增强日常活动能力,降低术后认知障碍的发生,提高患者生活质量及护理满意度,值得推广。
Objective To investigate the application effect of auricular acupoint pressing combined with sleep intervention in postoperative cognitive impairment(POCD)in elderly patients with orthopedics.Methods From November 2020 to November 2021,a total of 60 patients who underwent surgical treatment in the department of orthopedics of Shangrao People's Hospital were randomly divided into two groups by odd and even number.The control group of 30 cases received routine nursing,and the observation group of 30 cases received auricular acupoint pressing combined with sleep intervention.The clinical symptom scores,the incidence of POCD,the quality of life and nursing satisfaction were compared between the two groups before and after the intervention.Results On the 3rd and 7th days after operation,the MMSE and ADL of the observation group were significantly higher than those of the control group,and the PSQI was significantly lower than that of the control group.The incidence of POCD in observation group was significantly lower than that in control group at 3 d and 7 d after operation.The scores of quality of life and nursing satisfaction in the observation group were significantly higher than those in the control group.Conclusion The application of auricular acupoint pressing combined with sleep intervention in the postoperative cognitive impairment of orthopedics elderly patients can significantly improve the sleep quality of the patients,enhance the ability of daily activities,reduce the occurrence of postoperative cognitive impairment,and improve the quality of life and nursing satisfaction of the patients.It is worth promoting.
