4周跑台运动对糖尿病大鼠心肌缺血再灌注损伤的影响

Effects of 4-Week Treadmill Exercise on Myocardial Ischemia-Reperfusion Injury in Diabetic Rats.

摘要 目的探讨4周跑台运动对糖尿病心肌缺血再灌注损伤(IRI)产生的影响以及通过AMPK-mTOR信号通路调节糖代谢。方法6~8周龄雄性SD大鼠适应性喂养1周,禁食12 h后,一次性腹腔注射1%链脲佐菌素柠檬酸缓冲液60 mg/kg制备糖尿病大鼠模型,随机分为糖尿病+假手术组(DM+Sham组)、糖尿病+心肌缺血再灌注损伤组(DM+IRI组)、糖尿病+心肌缺血再灌注损伤+运动组(DM+IRI+E组),每组6只。DM+IRI+E组进行4周跑台运动训练。DM+IRI组和DM+IRI+E组大鼠予结扎左冠状动脉前降支30 min,恢复灌注120 min以制备心肌IRI模型;DM+Sham组大鼠只开胸,不结扎冠状动脉。比较三组大鼠空腹血糖(FBG)、空腹胰岛素水平(FINS)、胰岛素敏感指数(ISI)和胰岛素抵抗指数(HOMA-IR)、肌酸激酶同工酶(CK-MB)、心肌肌钙蛋白I(cTnI)、心肌肌钙蛋白T(cTnT)、心肌梗死面积、己糖激酶(HK)活性、心肌组织AMPK/mTOR表达水平。结果DM+IRI组大鼠与DM+Sham组相比,CK-MB、cTnI、cTnT、心肌梗死面积明显增高(P<0.01或P<0.05)。与DM+IRI组相比,DM+IRI+E组FBG、FINS、HOME-IR、CK-MB、cTnI、cTnT、心肌梗死面积明显降低(P<0.01或P<0.05),ISI、HK活性、心肌组织AMPK、p-AMPK表达均明显升高(P<0.01或P<0.05),p-mTOR水平下降(P<0.05),mTOR水平有下降趋势(P>0.05)。结论4周跑台运动可以通过AMPK-mTOR调控糖代谢、改善胰岛素抵抗发挥对糖尿病IRI心肌的保护作用。 Objective To investigate the effects of 4-week treadmill exercise on myocardial ischemia-reperfusion injury(IRI)in diabetes mellitus and the glucose-regulating metabolism of the AMPK-mTOR signaling pathway.Methods Male SD rats aged 6~8 weeks were fed adaptively for one week and fasted for 12 hours.The diabetic rat model was prepared by injecting 60 mg/kg of 1%streptozotocin citrate buffer into the abdomen.Those diabetic rats were randomly divided into sham operation group(DM+Sham group),diabetes with myocardial ischemia-reperfusion injury group(DM+IRI group),and diabetes with myocardial ischemia-reperfusion injury+exercise group(DM+IRI+E group).And there were 6 rats in each group.DM+IRI+E group underwent treadmill exercise training for 4 weeks.The rats in the DM+IRI group and DM+IRI+E group were ligation the anterior descending branch of the left coronary artery for 30 min and restored perfusion for 120 min to prepare the myocardial IRI model.In the DM+Sham group,only thoracotomy was performed without coronary artery ligation.Fasting blood glucose(FBG),fasting insulin level(FINS),insulin sensitivity index(ISI),insulin resistance index(HOMA-IR),creatine kinase isoenzyme(CK-MB),cardiac troponin I(cTnI),cardiac troponin T(cTnT),myocardial infarction size,hexokinase(HK)activity,the expression level of AMPK/mTOR in myocardial tissue of the three groups were compared.Results Compared with the DM+Sham group,CK-MB,cTnI,cTnT,and myocardial infarction area in the DM+IRI group were significantly increased(P<0.01 or P<0.05).Compared with the DM+IRI group,FBG,FINS,HOME-IR,CK-MB,cTnI,cTnT,and myocardial infarction area in the DM+IRI+E group were significantly decreased(P<0.01 or P<0.05).ISI,the activities of HK and the expressions of AMPK and p-AMPK in myocardial tissue were significantly increased(P<0.01 or P<0.05),while the level of p-MTOR was decreased(P<0.05),and the level of mTOR had a downward trend(P>0.05).Conclusion 4-week treadmill exercise can regulate glucose metabolism and improve insulin resistance through AMPK-mT
