摘要
目的:比较防旋股骨近端髓内钉(PFNA)与新一代髓内钉(InterTAN)在治疗股骨转子间骨折时术中指标和术后疗效的差异。方法:依据检索策略检索发表在Pubmed、Embase、Cinahl、Cochrane Library、中国知网、万方等数据库中关于PFNA和InterTAN治疗股骨转子间骨折的文献,共检索到相关文献1426篇,并最终纳入12篇文献。对文献依据Cochrane系统评价方法学进行质量评价,提取数据后用Review manager 5.4软件进行Meta分析。结果:PFNA组手术时间[95%CI(-14.98,-1.55),P=0.02]、术中透视时间[95%CI(-124.63,-33.11),P<0.001]、术中失血量[95%CI(-34.31,-9.66),P<0.001]较InterTAN组具有明显优势,其差异具有统计学意义;术后并发症发生率比较,InterTAN组较PFNA组具有明显优势[95%CI(1.54,3.05),P<0.001];术后疗效比较,PFNA组与InterTAN组差异无统计学意义。结论:手术治疗股骨转子间骨折时,PFNA具有手术时间短、术中透视时间及术中出血少的优点,而InterTAN较PFNA并发症发生率低。
Objective:To compare the differences between intraoperative indexes and postoperative efficacy of Proximal femoral nail antirotation(PFNA)and Xerox's new generation Intertrochanteric antegrade nail(InterTAN)in the treatment of intertrochanteric fractures.Methods:According to the search strategy,the literature on PFNA and InterTAN in the treatment of intertrochanteric fracture of the femur was searched in Pubmed,Embase,Cinahl,Cochrane Library,CNKI,Wanfang,and other databases,and a total of 1426 relevant literature was retrieved,and finally 12 articles were included.The quality of the literature was assessed according to the Cochrane systematic review methodology,and the data were extracted and then subjected to Meta-analysis using Review Manager 5.4 software.Results:For the surgical treatment of intertrochanteric fracture,the PFNA group had obvious advantages over the InterTAN group with surgical time[95%CI(-14.98,-1.55),P=0.02],intraoperative fluoroscopic time[95%CI(-124.63,-33.11),P<0.001],and intraoperative blood loss[95%CI(-34.31,-9.66),P<0.001]compared with the InterTAN group.In terms of complications,the InterTAN group had a significant advantage over the PFNA group[95%CI(1.54,3.05),P<0.001].There was no statistically significant difference between the PFNA group and the InterTAN group in terms of postoperative efficacy.Conclusions:In the treatment of intertrochanteric fractures,PFNA has the advantages of short operation time,intraoperative fluoroscopy time,and less intraoperative bleeding,while InterTAN has a lower postoperative complication rate than PFNA.
