血浆microRNA联合超声心动图与急性心肌梗死诊断的相关性研究 被引量:1

摘要 目的通过检测急性心肌梗死(AMI)患者血浆中microRNA-98-5p(miR-98-5p)、microRNA-499a-5p(miR-499a-5p)的表达情况,并分析二者联合超声心动图所测左室射血分数(LVEF)作为AMI早期诊断指标的可行性。方法选择2018年11月—2022年1月在包头市中心医院住院的45例AMI且发病到入院时间在12小时内的患者为AMI组,另取同期住院的31例非心梗患者作为对照组。两组患者均通过高通测序法检测血浆miR-98-5p、miR-499a-5p表达情况,并用超声心动图检测LVEF值。比较两组患者血浆miR-98-5p、miR-499a-5p及LVEF的组间差异,并通过绘制受试者工作特征曲线,分析血浆miR-98-5p、miR-499a-5p、LVEF、肌钙蛋白(cTnI)对AMI的诊断价值。结果LVEF及miR-98-5p的表达量在AMI组低于对照组(P<0.05);miR-499a-5p、cTnI在AMI组的表达量显著高于对照组(P<0.001)。miR-98-5p、miR-499a-5p、LVEF单独及联合诊断AMI的AUC分别为0.942、0.713、0.803、0.959;cTnI诊断AMI的AUC为0.856。结论miR-98-5p、miR-499a-5p、LVEF对AMI具有早期诊断的潜在价值,三者联合诊断价值更高,且三者联合诊断价值优于cTnI。 Objective To detect the expression of microRNA-98-5p(miR-98-5p)and microRNA-499a-5p(miR-499a-5p)in the plasma of patients with acute myocardial infarction(AMI),and to analyze the feasibility of the two indicators combined with left ventricular ejection fraction(LVEF)measured by echocardiography as an early diagnostic indicator of AMI.Methods 45 patients with AMI who were hospitalized in Baotou Central Hospital from November,2018 to January,2022 and the time from onset to admission was within 12 hours were selected as the AMI group,and 31 non-myocardial infarction patients who were hospitalized during the same period were selected as the control group.The expressions of plasma miR-98-5p and miR-499a-5p were detected by high-throughput sequencing,and LVEF was detected by echocardiography.The differences in plasma miR-98-5p,miR-499a-5p and LVEF between the two groups were compared,and the diagnostic value of plasma miR-98-5p,miR-499a-5p,LVEF and cTnI for AMI was analyzed by drawing the receiver operating characteristic curve.Results The LVEF and expression level of miR-98-5p in AMI group were significantly lower than those in control group(P<0.05).The expression of miR-499a-5p and cTnI in AMI group were significantly higher than that in control group(P<0.001).The AUC of miR-98-5p,miR-499a-5p,LVEF alone and the combination in diagnosis of AMI were 0.942,0.713,0.803 and 0.959,respectively.The AUC of cTnI in the diagnosis of AMI was 0.856.Conclusions miR-98-5p,miR-499a-5p and LVEF have potential value for early diagnosis of AMI.The combined diagnostic value of the three of them is higher,and the combined diagnostic value of the three indicators is better than cTnI.
