摘要
目的从炎症因子水平角度探讨电针足三里、梁门、三阴交治疗糖尿病性胃轻瘫(DGP)的可能作用机制。方法将50只SD大鼠随机分为空白组13只、造模组37只。造模组大鼠腹腔注射2%链脲菌素(STZ)溶液配合不规则高糖高脂饲料喂养8周制备DGP模型大鼠。将造模成功的30只大鼠随机分为模型组、电针组和胃复安组,每组10只。电针组取单侧“足三里”“梁门”“三阴交”进行电针刺激,每次15 min,每天1次,左右轮流取穴。胃复安组给予0.9%胃复安溶液10 ml/kg灌胃,空白组和模型组给予生理盐水10 ml/kg灌胃,均每天1次,干预5天休息2天为1个疗程,共3个疗程。每周1次检测大鼠血糖、尿糖,末次干预次日取材测胃排空率和小肠推进率,采用酶联免疫分析法(ELISA)测定胃窦组织中的NOD样受体热蛋白结构域相关蛋白3(NLRP3)炎症小体、白细胞介素18(IL-18)、白细胞介素1β(IL-1β)水平,HE染色观察胃窦组织病理形态。结果与空白组比较,模型组大鼠胃黏膜炎症反应明显,存在大量炎细胞浸润,胃排空率、小肠推进率均显著降低,血糖、尿糖、NLRP3、IL-1β、IL-18炎症因子水平显著升高(P<0.01)。与模型组比较,电针组和胃复安组大鼠胃黏膜有不同程度好转,炎细胞浸润显著减少,尿糖水平显著降低,胃排空率、小肠推进率均显著升高,电针组血糖水平降低(P<0.01)。与胃复安组比较,电针组大鼠胃窦组织NLRP3的水平明显升高(P<0.05)。结论电针“足三里”“梁门”“三阴交”能够改善DGP大鼠胃排空延迟,降低血糖水平,其机制可能与降低炎症因子水平,抑制炎症反应有关。
Objective To explore the possible mechanism of electroacupuncture(EA)at Zusanli(ST 36),Liangmen(ST 21)and Sanyinjiao(SP 6)in the treatment of diabetic gastroparesis(DGP)rats by regulating inflammatory factors.Methods Fifty SD rats were randomly divided into a blank group(n=13)and a modeling group(n=37).The modeling rats were injected intraperitoneally with 2%streptozotocin(STZ)solution and given irregular high sugar and high fat diet for 8 weeks to prepare DGP rat model.Thirty successfully modeled rats were randomly divided into model group,EA group and metoclopramide group,with 10 rats in each group.In the EA group,unilateral“Zusanli”,“Liangmen”and“Sanyinjiao”were needled,15 min each time,once daily,and acupoints on the left and right side were used in turn.In the metoclopramide group,10 ml/kg of 0.9%metoclopramide solution was given by gavage,while in the blank group and the model group,10 ml/kg of normal saline by gavage was given,once daily,five days of treatment and two days of rest as a course,for a total of three courses.The rats'blood glucose and urine glucose values were measured once every week,and on the second day of the last treatment,sample was collected to determine the gastric emptying rate and small intestine propulsion rate.The levels of NOD-like receptor thermoprotein domain-related protein 3(NLRP3)inflammasome,interleukin 18(IL-18),and interleukin 1β(IL-1β)in gastric antrum tissue were detected by enzyme-linked immunoassay(ELISA).Histopathological morphology of gastric antrum was observed by HE staining.Results Compared to the blank group,the model group had more obvious gastric mucosal inflammatory reaction and a large number of inflammatory cell infiltration;the gastric emptying rate and the small intestine propulsion rate were significantly lower in the model groups,while the levels of blood glucose,urine glucose,NLRP3,IL-1β,and IL-18 inflammatory factors were significantly higher(P<0.01).Compared to the model group,both the EA group and metoclopramide group had improved gast
