摘要 目的基于柯氏模型实施和评价急诊科室培训效果,促进急诊科室培训的规范化。方法以柯氏模型为指导,从反应层、学习层、行为层和结果层4个层面对急诊科室培训进行改革和评价,对比改革前后的护士对科室培训的满意度、认知度、参与情况、培训知识掌握情况及在临床工作中急诊急救知识的应用情况。结果基于柯氏模型进行改革的急诊科室培训模式,反应层护士对科室培训的认知度提升,对课程构建及培训方式的满意度、对培训重要性的认知度、1周内学习完成率、行为层护士的有效心得体会书写率、结果层的科室考核合格率、抽查培训掌握率、护士临床综合能力方面改革后均明显高于改革前(P均=0.000)。结论基于柯氏模型4个层面进行的科室培训改革,可以提升护士对科室培训的认知和参与率,且能提升护士的急诊急救知识和技能水平,将知识和技能有效应用与临床,满足急诊急救岗位临床需求。 Objective To implement and evaluate the effect of in-department training in emergency department based on the kirkpatrick model,and promote the standardization of in-department training in emergency department.Methods The reform and evaluation of in-department training in emergency department were carried out from the four levels of response level,learning level,behavior level and outcome level under the guidance of kirkpatrick model.The nurses'satisfaction,awareness,participation,knowledge of in-department training and the application of emergency knowledge in clinical work were compared before and after the reform.Results With the reformed intra-departmental training model in the emergency department based on the Koch model,the nurses'awareness of intra-departmental training increased in the response tier,and the satisfaction with the curriculum construction and training methods,the awareness of the importance of training,the completion rate of learning within 1 week,the effective experience writing rate of nurses in the behavioral tier,the passing rate of intra-departmental assessment in the outcome tier,the mastery rate of sampling training,and the nurses'clinical general competence were significantly higher after the reform than before the reform(all P=0.000).Conclusion The intramural training reform based on the four aspects of the kirkpatrick model can improve the cognition and participation rate of nurses in the intramural training,improve the knowledge and skills of nurses in emergency and first aid,effectively apply the knowledge and skills to the clinic,and meet the clinical needs of emergency and first aid positions.
