摘要
目的:构建一种依替米星应用量化评价体系,为医院管理部门制定临床药物治疗管理决策提供依据。方法:采用属性层次模型(AHM)赋权法对制定的各项评价指标进行赋权,再使用逼近理想解排序法(TOPSIS)对北京老年医院2021年1-6月应用依替米星注射液的159例病例进行回顾性分析并评价。结果:在评价体系中纳入12项评价依替米星应用合理性的指标,其中权重较高的有适应证、用药疗程、给药剂量与频次、疗效监测(权重分别为0.159、0.105、0.101、0.099)。使用这种评价方法得到的结果与传统处方点评方法相比一致率为94.97%。结论:AHM-TOPSIS法将多个指标具体量化并且结合起来,可用于依替米星药物利用的评价。评价结果显示,该院依替米星用药较为合理,但在适应证、用药疗程、病原学培养、配伍禁忌及相互作用等方面需进一步加强管理。
Objective A quantitative evaluation system of etimicin application was established to provide a basis for hospital administration to make clinical drug therapy management decisions.Methods Firstly,the attribute hierarchy model(AHM)weighting method was used to weight the evaluation indicators.Then,the weighted approximation ideal solution ranking method(TOPSIS)was used to retrospectively analyze and evaluate the patients who were treated with etimicin injection in Beijing Geriatric Hospital from January 2021 to June 2021.Results The established evaluation system included 12 evaluation indicators,among which indications,medication duration,dose and frequency of administration,and efficacy monitoring accounted for high weights(weights were 0.159,0.105,0.101,0.099,respectively).Among the 159 cases,27 cases(16.98%)were closer to the optimal solution than 80%,106 cases(66.67%)were between 60%and 80%,and 26 cases(16.35%)were closer than 60%.The results obtained by this evaluation method were 94.97%consistent with those obtained by traditional prescription review method.Conclusion AHM-TOPSIS method quantifies and combines multiple indicators,which can be used to evaluate the utilization of etimicin.The evaluation results showed that the drug use in the hospital was reasonable,but further management should be strengthened in indications,course of drug use,etiological culture,incompatibility and interaction.
