BiPAP无创呼吸机鼻导管、面罩供氧应用于COPD合并Ⅱ型呼吸衰竭患者的效果观察 被引量:5

摘要 目的探究双水平气道正压(Bilevel Positive Airway Pressure,BiPAP)无创呼吸机鼻导管、面罩供氧两种模式于慢性阻塞性肺疾病(Chronic Obstructive Pulmonary Disease,COPD)合并Ⅱ型呼吸衰竭患者的应用效果。方法将2020年4月—2021年4月本院收治的104例COPD合并Ⅱ型呼吸衰竭患者采用随机数字表法分为观察组、对照组,各52例,分别采取鼻导管、常规面罩供氧方式进行治疗干预。比较两组治疗总有效率、血氧分压(PaO_(2))、血二氧化碳分压(PaCO_(2))、用力肺活量(FVC)、一秒率(FEV_(1)/FVC)、呼气峰流量(PEF)、气道压(Paw)、COPD生活质量调查问卷(SGRQ)评分以及治疗后并发症发生率。结果观察组治疗后有效率显著高于对照组患者(P<0.05);两组治疗后的PaO_(2)、PaCO_(2)、FVC、PEF、Paw%、FEV_(1)/FVC及各项SGRQ评分相比治疗前均显著改善(P<0.05),且观察组优于对照组同期(P<0.05);观察组治疗后并发症发生率为21.54%,对照组的并发症发生率为34.62%,并发症发生率比较差异无统计学意义(P>0.05)。结论BiPAP无创呼吸机联合鼻导管供氧方式更有助于改善血气指标、肺功能以及预后生活质量,对于COPD合并Ⅱ型呼吸衰呼吸功能的改善以及临床症状的缓解具有较高的应用价值。 Objective To explore the clinical effect of bilevel positive airway pressure(BiPAP) non-invasive ventilator nasal cannula and mask oxygen supply in patients with chronic obstructive pulmonary disease(COPD) complicated with type II respiratory failure. Methods A total of 104 patients with COPD complicated with type II respiratory failure admitted to hospital from April 2020 to April 2021 were divided into observation group and control group, 52 cases in each group, respectively by nasal catheter and conventional mask oxygen supply. Total effective rate, partial blood oxygen pressure(PaO_(2)), partial blood carbon dioxide pressure(PaCO_(2)), forced vital capacity(FVC), percentage of forced expiratory volume in FVC(FEV_(1)/FVC), peak expiratory flow(PEF), airway opening pressure(Paw) and COPD quality of life questionnaire(SGRQ) were recorded before and after treatment between the 2 groups. Also, the incidence of complications after treatment were compared. Results The effective rate in the observation group was significantly higher than that in the control group(P<0.05).Also, PaO_(2), PaCO_(2), FVC, PEF, Paw%, FEV_(1)/FVC and SGRQ scores of both groups were significantly improved after treatment compared with before treatment(P<0.05). The above indexes in the observation group were better than those in the control group at the same period after treatment(P<0.05). The incidence of complications after treatment was 21.54% in the observation group and 34.62% in the control group with no significant difference(P<0.05). Conclusion BiPAP non-invasive ventilator combined with nasal catheter oxygen supply is more conducive to blood gas indicators, lung function and prognostic quality of life in patients with COPD combined with type II respiratory failure.
