无创呼吸机联合噻托溴铵对COPD合并呼吸衰竭患者动脉血气分析及肺功能的影响 被引量:1

Effects of Non-Invasive Ventilator Combined with Tiotropium Bromide on Arterial Blood Gas Analysis and Pulmonary Function in COPD Patients with Respiratory Failure

摘要 目的探讨无创呼吸机联合噻托溴铵对慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者动脉血气分析和肺功能的影响.方法选取2020年2月—2022年1月我院收治的84例COPD合并呼吸衰竭患者为研究对象,按随机数字表法分为两组,每组42例.对照组采用鼻导管吸氧联合噻托溴铵治疗,观察组采用无创呼吸机联合噻托溴铵治疗,均持续治疗2周.对比两组的动脉血气分析指标、肺功能指标、恢复情况及不良反应发生率.结果治疗前,两组的各项动脉血气分析指标及肺功能指标比较,组间差异无统计学意义(P>0.05);治疗后,观察组的动脉氧分压、血氧饱和度、最大呼吸峰流速、用力肺活量、第1秒用力呼气容积、1秒率均优于对照组,二氧化碳分压低于对照组,组间差异有统计学意义(P<0.05).观察组的ICU住院时间及总住院时间分别为(8.06±1.44)d、(17.76±2.01)d,均短于对照组,组间差异有统计学意义(P<0.05).两组的不良反应发生率比较,差异无统计学意义(P>0.05).结论无创呼吸机联合噻托溴铵能增强COPD合并呼吸衰竭治疗效果,有效改善患者动脉血气分析指标与肺功能指标,不良反应少,值得临床推广使用. Objective To investigate the effect of non-invasive ventilator combined with tiotropium bromide on blood gas analysis and pulmonary function in patients with chronic obstructive pulmonary disease(COPD)complicated with respiratory failure.Methods A total of 84 patients with COPD complicated with respiratory failure admitted to our hospital from February 2020 to January 2022 were selected as the study subjects.They were divided into two groups according to the random number table method,with 42 cases in each group.The control group was treated with nasal catheter oxygen inhalation and tiotropium bromide,and the observation group was treated with non-invasive ventilator combined with tiotropium bromide.Both groups were treated for 2 weeks.The arterial blood gas analysis indexes,pulmonary function indexes,recovery and incidence of adverse reactions were compared between the two groups.Results Before treatment,there were no significant differences in arterial blood gas analysis indexes and pulmonary function indexes between the two groups(P>0.05);after treatment,the arterial partial pressure of oxygen,blood oxygen saturation,maximum peak expiratory flow rate,forced vital capacity,forced expiratory volume in one second and rate in one second in the observation group were better than those in the control group,and the partial pressure of carbon dioxide was lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).The ICU length of stay and the total length of stay in the observation group were(8.06±1.44)d and(17.76±2.01)d,which were shorter than those in the control group,and the differences between the two groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Non-Invasive ventilator combined with tiotropium bromide can enhance the treatment effect of COPD complicated with respiratory failure,effectively improve the arterial blood gas analysis and pulmona
