摘要
目的分析磁共振引导聚焦超声对特发性震颤患者身体活动和认知水平的影响。方法以2020年1月-2022年1月苏州科技城医院收治的86例特发性震颤患者为研究对象,患者入院后由责任护士告知常规药物和高强度聚焦超声治疗风险和收益,将43例自愿采用常规药物治疗的患者设为对照组,43例自愿采用常规药物联合磁共振引导聚焦超声治疗的患者设为观察组。比较两组患者治疗前1 d和治疗完成后1 d的下肢Fugl-Meyer运动功能评分、Berg平衡量表评分、睁眼站立及闭眼站立时重心移动速度、脚-前-后站立时重心移动速度及动态稳定时间、蒙特利尔认知评估量表(MoCA)评分和简易精神状态检查量表(MMSE)评分情况。结果与对照组比较,治疗后观察组患者下肢Fugl-Meyer运动功能评分、Berg平衡量表评分、MoCA评分和MMSE评分均升高,睁眼站立及闭眼站立时重心移动速度、脚-前-后站立时重心移动速度及动态稳定时间均降低,差异有统计学意义(P<0.05)。结论磁共振引导聚焦超声能够在一定程度上改善特发性震颤患者的静态及动态平衡能力,提升下肢运动功能及认知水平。
Objective To analyze the effect of focused ultrasound guided by magnetic resonance on the physical activity and cognitive level in patients with idiopathic tremor.Methods A total of 86 patients with idiopathic tremor who were treated in the hospital from January 2020 to January 2022 were included in the study.After admission,the responsible nurse informed the patients about the risks and benefits of conventional drugs and high intensity focused ultrasound treatment.43 patients volunteered to use conventional drugs for the treatment and were assigned to the control group.The remaining 43 patients volunteered to use conventional drugs combined with magnetic resonance guided focused ultrasound treatment and were assigned to the observation group.The lower limb Fugl-Meyer motor function score,the Berg balance scale score,the center of gravity movement speed when standing with eyes open and closed,the center of gravity movement speed and dynamic stabilization time when standing with feet front back,the Montreal Cognitive Assessment Scale(MoCA)score,and the Simplified Mental State Examination Scale(MMSE)score were compared between the two groups of the patients on the first day before and the first day after the treatment.Results Compared with the control group,the Fugl-Meyer motor function score,Berg balance scale score,MoCA score and MMSE score of the lower extremities in the observation group were increased after the treatment,and the center of gravity movement speed of the observation group when standing with eyes open and eyes closed,the center of gravity movement speed of the foot-front-back standing and the dynamic stability time were decreased.The difference was significant(P<0.05).Conclusion MRI guided focused ultrasound can improve the static and dynamic balance ability of the patients with idiopathic tremor,and improve the motor function and cognitive level of lower limbs.
