色差仪测量挫伤颜色的时序性变化

Examination of color changes during the process of bruising by a colorimeter

摘要 目的探究挫伤皮肤颜色变化的规律,寻找合适指标对挫伤形成时间和初始程度进行推断。方法采用负压吸引的方式,在41名志愿者双侧前臂内侧模拟不同程度的皮肤挫伤区域。伤后前3 d每0.5 d使用色差仪测量,3 d后每1 d进行测量,直至挫伤区域肉眼难以分辨色差。对ΔL^(*)、Δa^(*)、Δb^(*)、ΔE^(*)_(ab)、ΔC^(*)、Δh 6个色差指标进行曲线拟合。对t_(a峰)、t_(b负)、t_(b峰)、t_(C峰)、t_(总)5个特征性时间指标,用曼-惠特尼U检验比较性别差异,用配对样本均数t检验分析挫伤程度差异。结果Δh指标的回归方程拟合程度最好(R^(2)=0.6395)。t_(a峰)、t_(b负)、t_(b峰)、t_(C峰)、t_(总)5个指标均与性别无关(P>0.05),t_(a峰)、t_(b峰)、t_(总)3个指标以及颜色的变化速率(ΔE^(*)_(ab)拟合曲线的斜率)与挫伤程度相关(P<0.05),但t_(b负)、t_(C峰)2个指标不受挫伤程度的影响(P>0.05)。t_(b负)出现在1.72±0.90 d,即Δb^(*)在19 h后为正值。结论Δh、Δb^(*)值可用于挫伤形成时间和初始程度的推断,Δb^(*)值的正负可以判断挫伤是否为19 h内形成。 Objective To explore the changes in the color of bruised skin,and to establish suitable indicators to infer the formation time and extent of bruising.Methods By vacuum suction,skin bruise models with different extent were established on the bilateral medial forearms of 41 volunteers.A colorimeter was used to measure the colorations every half day in the first three days after bruising,and every day after three days,until it was difficult to distinguish the chromatic aberration in the bruise area with naked eyes.The overall trends and regression curve were analyzed for six chromatic aberration indicators:ΔL*,Δa*,Δb*,ΔE*ab,ΔC andΔh.For the five characteristic time indicators ta peak,tb negative,tb peak,tC peak and ttotal,the Mann-Whitney U test was used to compare the gender differences,and the paired t-test was used to analyze the difference in the extent of bruising.Results The regression equation ofΔh index has the best fitting degree(R^(2)=0.6395).Furthermore,the five time indicators ta peak,tb negative,tb peak,tC peak and ttotal were not related to gender(P>0.05).Meanwhile,ta peak,tb peak,ttotal and the rate of color changes(the slope of theΔE*ab fitting curve)were related to the extent of bruising(P<0.05),while tb negative and tC peak,were not affected by the extent of bruising(P>0.05).Among them,the tb negative appeared at 1.72±0.90 days,which indicated that theΔb*value became positive 19 hours after bruising.Conclusion The values ofΔh andΔb*can be used to infer the formation time and initial extent of contusion,and the positive or negative value ofΔb*can be used to judge whether the contusion is formed within 19 hours.
