摘要
目的 探索永久性肠造口患者的延续护理干预策略,以提升患者自我护理能力及生活质量。方法 选取2021年1月至2022年6月于上海市某综合医院胃肠外科行腹会阴联合直肠癌根治术(Miles术)并留置永久性肠造口的患者为研究对象,将其随机分为观察组(n=40)和对照组(n=40)。对照组接受常规护理和造口护理门诊随访,观察组在此基础上采取多学科一体化延续护理模式。比较两组患者出院3个月后自我护理能力量表与造口生命质量量表评分,以及局部出血、水肿、回缩、机械性损伤及皮炎等造口相关并发症的发生情况。结果 出院3个月后,观察组患者的自我护理能力评分及生活质量总评分均高于对照组(均P<0.05);观察组患者造口相关并发症发生率为12.5%,明显低于对照组的32.5%(P<0.05)。结论 多学科一体化延续护理可有效提高永久性肠造口患者术后自我护理能力,降低造口相关并发症的发生率,有助于提高患者生活质量。
Objective To explore transitional care intervention strategies for patients with permanent stomas,so as to improve their self⁃care agency and quality of life.Methods From January 2021 to June 2022,patients who underwent abdominoperineal resection(Miles resection)in the Gastrointestinal Surgery Department of a general hospital in Shanghai were selected.They were randomly divided into the observation group(n=40)and the control group(n=40).The control group was given routine care and stoma care at outpatient follow⁃up,while the observation group was given multidisciplinary integrated transitional care on this basis.The scores of the Exercise of Self⁃Care Agency Scale and Stoma⁃QOL,and the incidence of stoma⁃related complications such as local bleeding,edema,retraction,mechanical injury and dermatitis,were compared between the two groups three months after discharge.Results Three months after discharge,the scores of self⁃care agency and quality of life in the observation group were higher than those in the control group(both P<0.05);the incidence of stoma⁃related complications in the observation group was 12.5%,which was significantly lower than that in the control group(32.5%,P<0.05).Conclusion Multidisciplinary integrated transitional care can effectively improve the postoperative self⁃care agency of patients with permanent stomas,reduce the incidence of stoma⁃related complications and improve their quality of life.
