摘要
目的:探讨肿瘤转移相关基因1(MTA1)、叉头翼螺旋转录因子3(FOXP3)在甲状腺乳头状癌(PTC)淋巴转移患者中的作用及临床意义。方法:选取手术治疗的111例PTC患者为研究对象,收集患者的癌组织及癌旁组织标本,采用免疫组织化学染色法观察组织中MTA1、FOXP3表达情况;采用多因素Logistic回归风险模型分析影响PTC淋巴转移的因素。结果:111例PTC组织标本中,MTA1的阳性表达率(MTA1阳性例数/癌组织总例数)为78.38%(87/111)高于癌旁组织17.12%(19/111),FOXP3在PTC组织中的阳性表达率(FOXP3阳性例数/癌组织总例数)为70.27%(78/111)高于癌旁组织13.51%(15/111),两组比较差异有统计学意义(均P<0.05);Spearman等级相关分析结果显示MTA1与FOXP3表达呈正相关(r=0.532,P=0.000);PTC患者癌组织中MTA1与FOXP3表达与包膜侵犯、淋巴结转移、TNM分期有关(P<0.05);单因素分析结果显示淋巴转移患者与未转移患者在性别、病灶数、包膜侵犯、TNM分期、脉管侵犯、MTA1及FOXP3等病理特征方面比较有统计学差异(均P<0.05);多因素Logistic回归分析结果显示性别、病灶、TNM分期、包膜侵犯及MTA1、FOXP3表达均是影响PTC淋巴转移的危险因素(均P<0.05)。结论:MTA1、FOXP3在PTC患者癌组织中表达均升高,其表达水平与包膜侵犯、淋巴结转移、TNM分期有关,可作为评估PTC淋巴转移的有效指标。
Objective:To investigate the role and clinical significance of tumor metastasis-associated gene 1(MTA1)and forkhead box protein 3(FOXP3)in patients with lymph node metastasis of thyroid papillary carcinoma(PTC).Methods:A total of 111 patients with PTC who underwent surgical treatment were selected as the study subjects,cancer tissue and adjacent tissue samples of patients were collected,the expressions of MTA1 and FOXP3 were observed by immunohistochemistry.Multivariate Logistic regression risk model was used to analyze the factors affecting lymphatic metastasis of PTC.Results:Among 111 PTC lymphoid tissues,the positive expression rate of MTA1(number of MTA1 positive cases/total number of cancer tissues)was 78.38%(87/111),which was higher than 17.12%(19/111)in adjacent tissues(P<0.05);the positive expression rate of FOXP3 in PTC tissues(number of FOXP3 positive cases/total number of cancer tissues)was 70.27%(78/111),which was higher than 13.51%(15/111)in adjacent tissues(P<0.05).Spearman rank correlation analysis showed that the expressions of MTA1 and FOXP3 were positively correlated(r=0.532,P=0.000).The expressions of MTA1 and FOXP3 in cancer tissues of PTC patients were correlated with capsule invasion,lymph node metastasis and TNM stage(all P<0.05).Univariate analysis showed that there were obvious differences between patients with lymphatic metastasis and patients without lymphatic metastasis in terms of gender,number of lesions,capsule invasion,TNM stage,vascular invasion,MTA1 and FOXP3(all P<0.05).Multivariate Logistic regression analysis showed that gender,lesion,TNM stage,capsule invasion and the expressions of MTA1 and FOXP3 were risk factors for PTC lymphatic metastasis(all P<0.05).Conclusion:The expressions of MTA1 and FOXP3 in cancer tissues of PTC patients are elevated.Their expression levels are related to capsule invasion,lymph node metastasis and TNM stage,which can be used as effective indicators to evaluate the lymphatic metastasis of PTC.
