摘要
目的探讨循证护理联合复合呼吸操对老年慢性阻塞性肺部疾病(COPD)患者肺功能的影响。方法选取2018年6月-2020年4月中国人民解放军联勤保障部队第九〇〇医院梅峰院区收治的62例老年COPD患者,按照双盲抽签法进行分组,分为对照组和观察组,每组均有31例患者,对照组采用常规护理,观察组在对照组的基础上采用循证护理联合复合呼吸操护理,均治疗8个月。比较两组患者干预前后肺功能指标与动脉血气水平及依从性。结果干预后8个月,两组患者用力肺活量(FVC)、第一秒用力呼气量(FEV1)、每分钟最大通气量(MVV)以及第一秒用力呼气量占用力肺活量之比值(FEV1/FVC)均升高,且观察组高于对照组(P<0.05);两组患者动脉血氧饱和度(SaO2)、动脉氧分压(PaO2)水平均升高,观察组高于对照组,动脉血二氧化碳分压(PaCO_(2))水平均降低,观察组低于对照组(P<0.05);观察组总依从性高于对照组(P<0.05)。结论应用循证护理联合复合呼吸操的干预模式可以提升老年COPD患者肺功能,改善动脉血气水平,提高患者锻炼的依从性。
Objective To explore the effect of evidence-based care combined with compound breathing exercises on pulmonary function in elderly patients with chronic obstructive pulmonary disease(COPD).Methods Sixty-two cases of elderly COPD patients admitted to our hospital from June 2018 to April 2020 were selected and grouped according to the double-blind lottery method,divided into control group and observation group,each group had 31 patients,the control group used conventional care,the observation group used evidence-based care combined with compound respiratory exercise care on the basis of the control group,both treated for 8 months.Pulmonary function indexes and arterial blood gas levels and compliance were compared between the two groups before and after the intervention.Results At 8 months after the intervention,the exertional spirometry(FVC),first second exertional expiratory volume(FEV1),maximum random ventilation(MVV),and the ratio of first second exertional expiratory volume to exertional spirometry(FEV1/FVC)were higher in both groups,and the observation group was higher than the control group(P<0.05);the arterial oxygen saturation(SaO2)and arterial partial pressure of oxygen(PaO2)levels were higher in both groups,and the observation group was higher than the control group.The arterial oxygen saturation(SaO2)and arterial oxygen partial pressure(PaO2)levels increased in both groups and were higher in the observation group than in the control group,and the arterial carbon dioxide partial pressure(PaCO_(2))levels decreased in both groups and were lower in the observation group than in the control group(P<0.05);the total compliance was higher in the observation group than in the control group(P<0.05).Conclusion Applying the intervention mode of evidence-based care combined with composite respiratory exercises can improve the pulmonary function of elderly COPD patients,improve the arterial blood gas level,and increase the patient's exercise compliance.
