摘要
目的探讨术前实验室衰弱指数(FI-Lab)与老年患者髋部骨折术后并发症的相关性。方法回顾性分析124例因髋部骨折行手术的老年患者的临床资料,其中男32例,女92例;年龄65~102岁,平均(82.79±7.53)岁。根据术前FI-Lab评分分为衰弱组(n=78)和非衰弱组(n=46)。比较两组患者术后并发症的发生情况并分为无并发症组和有并发症组,分析FI-Lab与老年患者髋部骨折术后并发症的相关性。结果共有25例(20.16%)患者发生术后并发症。衰弱组和非衰弱组术后并发症发生率分别为28.21%(22/78)和6.52%(3/46),差异有统计学意义(P<0.05)。衰弱组肺部感染发生率为12.82%(10/78),高于非衰弱组的2.17%(1/46),差异有统计学意义(P<0.05)。与无并发症组比较,有并发症组术前衰弱比例更高,术前白细胞、中性粒细胞水平更高(P<0.05)。多因素logistic回归分析结果显示,FI-Lab评分≥0.3可能是术后并发症的独立危险因素(OR=5.203,95%CI 1.440~18.802,P=0.012)。结论FI-Lab评分≥0.3可能是老年患者髋部骨折术后并发症的独立危险因素,增加术后并发症的风险。
Objective To evaluate the association between preoperative laboratory frailty index(FI-Lab)scores and postoperative complications following hip fracture surgery in elderly patients.Methods Clinical data of 124 patients over 65 years old receiving hip fracture surgery were retrospectively analyzed.There were 32 males and 92 females with an average age of(82.79±7.53)years(65-102 years).Laboratory frailty index(FI-Lab)scores were generated to measure preoperative frailty,all were divided into FI-Lab scores≥0.3 group(n=78)and FI-Lab scores<0.3 group(n=46).Complications after operation were compared between the two groups.Regression analysis was used to assess the risk of frailty and postoperative complications.Results Twenty-five patients(20.16%)had postoperative complications.The total postoperative complication rates in the FI-Lab scores≥0.3 group and the FI-Lab scores<0.3 group was 28.21%(22/78)and 6.52%(3/46),respectively,and differences were statistically significant(P<0.05).Incidence of pulmonary infection in the FI-Lab scores≥0.3 group was 12.82%(10/78),which were significantly higher than that of the FI-Lab scores<0.3 group[2.17%(1/46)](P<0.05).Compared with the non-complication group,the incidence of frailty,the leukocyte and neutrophil counts were significantly higher than that of the FI-Lab scores<0.3 group(P<0.05).As shown by multifactorial logistic regression analysis,FI-Lab scores≥0.3(OR=5.203,95%CI:1.440-18.802,P=0.012)was independent risk factor of postoperative complications.Conclusion FI-Lab scores≥0.3 is an independent risk factor of postoperative complications following hip fracture surgery in elderly patients.FI-Lab scores≥0.3 will increase the risk of postoperative complications.
