摘要
目的:探讨托里通窦汤联合西药治疗慢性鼻窦炎合并过敏性鼻炎患者的临床疗效及对嗜酸性粒细胞计数(EOS)、嗜酸性粒细胞阳离子蛋白(ECP)的影响。方法:选取本院2018年6月~2021年9月96例肺脾气虚型慢性鼻窦炎合并过敏性鼻炎患者,随机分为观察组与对照组,每组48例患者,对照组给予常规西医治疗,观察组在对照组的基础上给予托里通窦汤治疗,比较两组临床疗效以及治疗前后患者临床症状、鼻腔体征[鼻内镜Lund-Kennedy评分、鼻窦CT Lund-Mackay评分、鼻腔鼻窦结局测试~20(SNOT-20)评分]、通气功能[第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、FEV1/FVC比值、峰值呼气流速(PEF)]、炎性指标[免疫球蛋白E(IgE)、EOS、ECP、白介素-1β(IL-1β)、白介素-17(IL-17)、肿瘤坏死因子-α(TNF-α)]水平。结果:两组临床治疗有效率比较(P>0.05);观察组治疗后鼻塞、鼻痒、喷嚏、流涕、头面部胀痛、嗅觉减退症状评分低于对照组(P<0.05);观察组治疗后Lund-Kennedy评分、Lund-Mackay评分、SNOT-20评分低于对照组(P<0.05);观察组治疗后FEV1、FVC、PEF水平高于对照组(P<0.05);观察组治疗后血EOS、ECP、IgE水平低于对照组,血清IL-1β、TNF-α、IL-17水平低于对照组(P<0.05)。结论:托里通窦汤联合西药治疗可有效缓解慢性鼻窦炎合并过敏性鼻炎患者临床症状及及鼻腔体征,改善通气功能,减轻机体炎症反应。
Objective:To explore the clinical curative effect of using Tuoli Tongdou Decoction combined with western medicine in the treatment of patients with chronic sinusitis and allergic rhinitis and its influences on eosinophil count(EOS) and eosinophil cationic protein(ECP).Methods:A total of 96 patients with chronic sinusitis and allergic rhinitis of lung-spleen Qi deficiency in the hospital were enrolled between June 2,018 and September 2,021.They were randomly divided into an observation group and a controlled group,48 cases in each group.The controlled group was treated with routine western medicine,while the observation group was treated with Tuoli Tongdou Decoction on the basis of the controlled group.The clinical curative effect,clinical symptoms,nasal signs[scores of nasal endoscopy Lund-Kennedy,sinus CT Lund-Mackay and the sino-nasal outcome test-20(SNOT-20)],ventilation function indexes[forced expiratory volume in 1 second(FEV1),forced vital capacity(FVC),FEV1/FVC,peak expiratory flow(PEF)] and inflammatory indexes[immunoglobulin E(IgE),EOS,ECP,interleukin-1β(IL-1β),interleukin-17(IL-17),tumor necrosis factor-α(TNF-α)] before and after treatment were compared between the two groups.Results:The difference in response rate of clinical treatment between the two groups was not statistically significant(P>0.05).After treatment,scores of clinical symptoms(nasal congestion,nasal itching,sneezing,runny nose,swelling pain of head and face,hyposmia) in the observation group were lower than those in the controlled group(P<0.05),scores of Lund-Kennedy,Lund-Mackay and SNOT-20 were lower than those in the controlled group(P<0.05),levels of FEV1,FVC and PEF were higher than those in the controlled group(P<0.05),levels of EOS,ECP and IgE were lower than those in the controlled group,and levels of serum IL-1β,TNF-α and IL-17 were lower than those in the controlled group(P<0.05).Conclusion:Tuoli Tongdou Decoction combined with western medicine can effectively relieve clinical symptoms and nasal signs in patients with chr
