多层螺旋CT与核磁共振成像在细微及隐匿性骨折中的诊断价值 被引量:2

Diagnostic Value of Multilayer Spiral CT and Nuclear Magnetic Resonance Imaging in Subtle and Occult Fractures

摘要 目的 比较多层螺旋CT(multisliecs helieal CT,MSCT)与核磁共振成像(magnetic resonance imaging,MRI)诊断细微与隐匿性骨折的临床价值。方法 选取2016年6月—2021年6月山丹县人民医院骨科收治的200例疑似细微与隐匿性骨折患者为研究对象,所有患者均开展MSCT与MRI检查,比较两种方法的诊断价值。结果 MSCT与MRI对各部位骨折的检出率比较,差异无统计学意义(P>0.05),但MRI总检出率为94.00%,明显高于MSCT的79.00%,差异有统计学意义(P<0.05);MRI诊断细微与隐匿性骨折的灵敏度为98.95%,阴性预测值为80.00%,均高于MSCT的83.16%与15.79%,差异有统计学意义(P<0.05);两种方法的特异度与阳性预测值比较,差异无统计学意义(P>0.05)。结论 MSCT与MRI诊断细微与隐匿性骨折的诊断价值均较高,但MRI的诊断价值更突出。 Objective To compare the clinical value of multilayer spiral CT(MSCT)and nuclear magnetic resonance imaging(MRI)in the diagnosis of subtle and occult fractures.Methods Two hundred patients with subtle and occult fractures admitted to the Shandan County People's Hospital orthopedic department from June 2016 to June 2021 were selected as the study subjects.MSCT and MRI were performed in all patients,and the diagnostic value of the two methods was compared.Results There was no statistically significant difference between MSCT and MRI in the detection rate of fractures at each site(P>0.05).However,the overall detection rate of MRI was 94.00%,which was significantly higher than that of MSCT 79.00%,and the difference was statistically significant(P<0.05).The sensitivity of MRI in diagnosing subtle and occult fractures was 98.95%,and the negative predictive value was 80.00%,which were higher than that of MSCT 83.16%and 15.79%,and the difference was statistically significant(P<0.05).There was no significant difference in the specificity and positive predictive value between the two methods(P>0.05).Conclusion The diagnostic value of both MSCT and MRI for the diagnosis of subtle and occult fractures is high,but the diagnostic value of MRI is more prominent.
