摘要
目的分析甘露糖结合凝集素相关丝氨酸蛋白酶2(MASP-2)基因多态性与2型糖尿病(T2DM)肺部感染的关联。方法收集2018年5月-2021年5月武汉市第一医院收治的274例T2DM患者临床资料,根据是否并发肺部感染分为并发肺部感染组80例和无肺部感染组194例,分析T2DM肺部感染患者病原菌分布情况,检测两组MASP-2基因rs2273346、rs6695096和rs72550870位点多态性,分析其与T2DM并发肺部感染的关系,并检测两组肺泡巨噬细胞人类白细胞抗原(HLA)-Ⅱ类抗原的表达。结果80例T2DM肺部感染患者共检出病原菌101株,其中革兰阴性菌占54.46%(55/101);肺部感染组MASP-2基因rs72550870位点AA基因型和A等位基因分布频率高于无肺部感染组(P<0.05),两组MASP-2基因rs2273346、rs6695096位点基因型和等位基因分布频率比较,无统计学差异;Logistic回归分析结果显示,rs72550870位点AA基因型是影响T2DM并发肺部感染的危险因素(P<0.05);肺部感染组携带MASP-2基因rs72550870位点AA基因型者HLA-DR、HLA-DQ抗原表达阳性占比均低于无肺部感染组(P<0.05)。结论T2DM肺部感染患者病原菌以革兰阴性菌为主,MASP-2 rs72550870位点AA基因型可能增加T2DM并发肺部感染风险,其作用机制可能与降低肺泡巨噬细胞HLA-Ⅱ类抗原表达有关。
OBJECTIVE To observe the association between mannan-binding lectin-associated serine protease 2(MASP-2)gene polymorphism and pulmonary infection in the patients with type 2 diabetes mellitus(T2DM).METHODS A total of 274 patients with T2DM who were treated in Wuhan No.1 Hospital from May 2018 to May 2021 were enrolled in the study and divided into the pulmonary infection group with 80 cases and the no pulmonary infection group with 194 cases according to the status of complication with pulmonary infection,the clinical data were collected from the patients,the distribution of pathogens isolated from the T2DM patients with pulmonary infection was observed.The polymorphisms at rs2273346,rs6695096 and rs72550870 loci of MASP-2 gene were detected,their association with the pulmonary infection was analyzed,and the expression levels of human leukocyte antigen(HLA)-Ⅱantigens in alveolar macrophages were detected.RESULTS Totally 101 strains of pathogens were isolated from the 80 T2DM patients complicated with pulmonary infection,54.46%(55/101)of which were gram-negative bacteria.The frequencies of AA genotype and A allele at rs72550870 locus of MASP-2 gene were higher in the pulmonary infection group than in the no pulmonary infection group(P<0.05).There were no significant differences in the frequencies of genotypes and alleles at rs2273346 locus and rs6695096 locus of MASP-2 gene between the two groups.The result of logistic regression analysis showed that the AA genotype at rs72550870 locus was the risk factor for the pulmonary infection in the patients with T2DM(P<0.05).The proportions of the patients who carried with AA genotype at rs72550870 locus of MASP-2 gene and were positive for expressions of HLA-DA and HLA-DQ were lower in the pulmonary infection group than in the no pulmonary infection group(P<0.05).CONCLUSION The gram-negative bacteria are dominant among the pathogens isolated from the T2DM patients with pulmonary infection.The AA genotype at rs72550870 locus of MASP-2 gene may increase the risk of pulmona
