鼻湿化高流量辅助通气对新生儿窒息血氧状态及预后的影响

摘要 目的探讨鼻湿化高流量辅助通气(HHFNC)对新生儿窒息血氧状态及预后的影响。方法选取2019年6月至2021年5月大连市妇女儿童医疗中心(集团)春柳院区新生儿科收治的84例窒息新生儿作为研究对象,按照随机数字表法分为对照组(42例)与观察组(42例),对照组采取常规鼻导管辅助通气治疗,观察组采取HHFNC治疗。比较两组患儿治疗后相关临床指标和并发症发生情况,同时比较治疗前后两组患儿窒息程度、血氧状态。结果观察组患儿抢救时间、持续辅助通气时间、呼吸恢复平稳时间及住院时间均短于对照组,治疗费用少于对照组,差异有统计学意义(P<0.05);观察组患儿治疗5、30 min及1 h后的阿氏(Apgar)评分均高于对照组,差异有统计学意义(P<0.05);观察组患儿动脉血氧分压(PaO_(2))、血氧饱和度(SaO_(2))及血液氢离子浓度指数(pH)值均高于对照组,动脉二氧化碳分压(PaCO_(2))低于对照组,差异有统计学意义(P<0.05);观察组患儿并发症总发生率为4.76%,低于对照组的21.43%,差异有统计学意义(P<0.05)。结论HHFNC治疗能有效改善患儿窒息程度和血氧状态,减少并发症发生情况,提高临床治疗效果。 Objective To investigate the effect of humidified high flow nasal cannula(HHFNC)on blood oxygen and prognosis of neonates with asphyxia.Methods A total of 84 asphyxia neonates admitted to the Department of Neonatology,Chunliu Branch of Dalian Women and Children's Medical Center(Group)from June 2019 to May 2021 were selected as the research objects.According to the random number table method,they were divided into control group(42 cases)and observation group(42 cases).The control group was treated with conventional nasal catheter assisted ventilation,and the observation group was treated with HHFNC.The related clinical indicators and complications after treatment were compared between the two groups,and the degree of asphyxia and blood oxygen status were compared between the two groups before and after treatment.Results The rescue time,continuous assisted ventilation time,respiratory recovery time and hospitalization time of the observation group were shorter than those of the control group,and the treatment cost was less than that of the control group,and the differences were statistically significant(P<0.05).The Apgar scores of the observation group after 5 min,30 min and 1 h of treatment were higher than those of the control group,and the differences were statistically significant(P<0.05).The arterial partial pressure of oxygen(PaO_(2)),blood oxygen saturation(SaO_(2))and blood hydrogen ion concentration index(pH)value of the observation group were higher than those of the control group,and the arterial partial pressure of carbon dioxide(PaCO_(2))was lower than that of the control group,and the differences were statistically significant(P<0.05).The total incidence of complications in the observation group was 4.76%,which was lower than 21.43%in the control group,and the difference was statistically significant(P<0.05).Conclusion HHFNC treatment can effectively improve the degree of asphyxia and blood oxygen status of children,reduce the incidence of complications,and improve the clinical treatment effect.
