摘要
目的 探讨血清和肽素、神经元特异性烯醇化酶(NSE)水平在预测新生儿缺氧缺血性脑病(HIE)远期神经发育中的意义.方法 选取2017年1月至2018年6月期间(在本院及复旦儿科)收治的HIE新生儿100例,根据血清和肽素与NSE水平变化将新生儿分为降低组(入院后3d较入院时血清和肽素或血清NSE水平降低,n=46)与未降低组(其余新生儿,n=54).比较两组新生儿3月龄、6月龄与12月龄时的神经发育水平.结果 降低组新生儿3月龄、6月龄与12月龄时DQ值明显高于未降低组(P<0.05).降低组新生儿3月龄、6月龄与12月龄时运动行为神经发育落后率明显低于于未降低组(P<0.05).经Pearson相关性分析显示,降低组新生儿早期血清和肽素、NSE水平差值与12月龄时DQ值呈正相关(r=0.514,0.602,均P=0.000).结论 血清和肽素、NSE水平在预测HIE远期神经发育中具有重要的意义,两者降低幅度越高,远期神经发育越佳.
Objective To study the significance in predicting long-term nerve development of serum copeptin,neurone specific enolase(NSE)levels for neonatal hypoxic ischemic encephalopathy(HIE).Methods 100 cases of neonatal HIE admitted in neonatology inpatient department of our hospital during the period of January 2017 and June 2018 were selected,according to the change of serum copeptin and NSE levels,neonatal were divided into lower group(3 d admission serum copeptin or NSE levels decreased compared with admission,n=46)and no-lower group(other newborn,n=54).The newborn 3 months,6 months and 12 months of neural development between the two groups were compared.Results DQ values when newborn 3 months,6 months and 12 months of the lower group were significantly higher than no-lower group,compared between the two groups with statistically significant differences(P<0.05).Delayed motor and neurobehavioral development when newborn 3 months,6 months and 12 months of the lower group were significantly lower than no-lower group(P<0.05).The Pearson correlation analysis showed that serum copeptin,NSE difference values in lower group were positively correlated with DQ values when newborn 12 months(r=0.514,0.602,P=0.000).Conclusion It has vital significance in predicting long-term nerve development of serum copeptin,NSE levels for neonatal HIE,both reduction higher,long-term neurological development better.
