摘要
目的比较输尿管软镜(FURL)、输尿管硬镜(URL)与微创经皮肾镜(mPCNL)治疗输尿管上段复杂结石(CUUC)的疗效。方法选取2019年1月至2021年6月该院收治的90例CUUC患者作为研究对象,根据治疗方法不同分为A、B、C 3组,每组各30例。A组实施FURL治疗,B组实施URL治疗,C组实施mPCNL治疗,比较3组患者的治疗效果、手术相关指标和术后并发症。结果A组和C组患者治疗有效率均高于B组,差异均有统计学意义(P<0.05);A组和C组患者治疗有效率比较,差异无统计学意义(P>0.05)。C组患者手术时间和住院时间均长于A组和B组,C组术中出血量均多于A组和B组,A组患者手术时间长于B组,差异均有统计学意义(P<0.05);A组和B组患者术中出血量和住院时间比较,差异均无统计学意义(P>0.05)。A组和B组患者并发症总发生率均低于C组,差异均有统计学意义(P<0.05);A组和B组患者并发症总发生率比较,差异无统计学意义(P>0.05)。结论相对于URL治疗,mPCNL、FURL治疗CUUC效果更加明显,FURL较mPCNL可减少患者术中出血量,缩短其康复时间,且术后并发症更少。
Objective To compare the efficacy of flexible ureteroscopy(FURL),rigid ureteroscopy(URL)and minimally invasive percutaneous nephroscopy(mPCNL)in the treatment of complex upper ureteral calculi(CUUC).Methods A total of 90 CUUC patients admitted to our hospital from January 2019 to June 2021 were selected,with 30 cases in each case.Group A was treated with FURL,group B was treated with URL,and group C was treated with mPCNL.The clinical efficacy,surgery-related indicators and postoperative complications were compared among the three groups after treatment.Results The treatment effective rates of group A and group C were higher than that of group B,with statistical significance(P<0.05).There was no significant difference on the treatment effective rate between group A and group C(P>0.05).The operation time and hospital stay of group C were longer than those of group A and group B,and the intraoperative blood loss of group C was more than those of group A and group B,the operation time of group A was longer than that of group B,with statistical significance(P<0.05).There was no significant difference on intraoperative blood loss and hospital stay between group A and group B(P>0.05).The total incidence of complications in group A and group B were lower than that in group C,the differences were statistically significant(P<0.05).There was no significant difference on the total incidence of complications between group A and group B(P>0.05).Conclusion Compared with URL therapy,mPCNL and FURL are more effective in the treatment of CUUC.Compared with mPCNL,FURL can reduce intraoperative blood loss,shorten the recovery time of patients,and have fewer postoperative complications.
