摘要
目的对比经皮肾镜与输尿管软硬镜治疗输尿管上段复杂结石的临床效果。方法选取2019年10月至2021年2月我院收治的90例输尿管上段复杂结石患者,随机分为对照组(n=45)和观察组(n=45)。对照组采用经皮肾镜治疗,观察组采用输尿管软硬镜治疗。比较两组的手术相关指标、结石清除率及炎性因子水平。结果观察组术中出血量少于对照组,手术时间、住院时间短于对照组(P<0.05)。观察组术后4周的结石清除率高于对照组(P<0.05);两组患者术后12周的结石清除率比较无统计学差异(P>0.05)。术后1周,两组的CRP、IL-6水平高于术前,但观察组CRP、IL-6水平低于对照组(P<0.05)。结论与经皮肾镜治疗相比,输尿管软硬镜治疗输尿管上段复杂结石可明显减少术中出血量,缩短患者的手术时间和住院时间,提高早期结石清除率,减轻机体炎性反应。
Objective To compare the clinical effects of percutaneous nephroscopy and ureteroscopy in the treatment of complicated upper ureteral calculi.Methods 90 patients with complicated upper ureteral calculi admitted to our hospital from October 2019 to February 2021 were selected and randomly divided into control group(n=45)and observation group(n=45).The control group was treated with percutaneous nephroscopy,and the observation group was treated with ureteroscopy.The operation related indicators,stone clearance rate and inflammatory factor levels were compared between the two groups.Results The intraoperative blood loss of the observation group was less than that of the control group,and the operation time and hospitalization time were shorter than those of the control group(P<0.05).The stone clearance rate at 4 weeks after operation of the observation group was higher than that of the control group(P<0.05);No statistical difference was found in the stone clearance rate at 12 weeks after operation between the two groups(P>0.05).1 week after operation,the CRP and IL-6 levels of the two groups were higher than those before operation(P<0.05);The CRP and IL-6 levels of the observation group were lower those of the control group(P<0.05).Conclusions Compared with percutaneous nephroscopy,ureteroscopy in the treatment of complicated upper ureteral calculi can significantly reduce the intraoperative blood loss,shorten the operation time and hospitalization time,improve the early stone clearance rate and reduce the body inflammatory reaction of patients.
