摘要
目的构建ICU患者谵妄预防分级管理方案,为ICU患者早期谵妄分级预防提供参考。方法通过检索国内外文献及开展半结构式访谈,拟订谵妄预防分级管理方案初稿。将方案进行2轮德尔菲专家函询,根据专家函询的意见,结合临床实践,对条目进行修改,并形成最终定稿。结果2轮函询有效问卷回收率分别为94.7%和100%,权威系数为0.91,肯德尔和谐系数为0.135~0.595,差异有统计学意义(P<0.01)。第2轮函询中,各条目变异系数为0~0.18,满分率为38.89%~100%。最终形成了基于早期谵妄预测模型的ICU患者谵妄预防分级管理方案,包括6个一级条目,19个二级条目,50个三级条目。结论基于早期谵妄预测模型构建的ICU患者谵妄预防分级管理方案具有科学性、可靠性、针对性及可操作性,可为今后ICU谵妄预防管理提供参考。
Objective To construct a graded management scheme for delirium prevention in ICU patients,and so as to provide a reference for the graded prevention of delirium in ICU patients.Methods The first draft of the hierarchical management program for delirium prevention was prepared by searching domestic and foreign literature and conducting semi-structured interviews.The scheme was carried out by two rounds of Delphi expert letter consultation,and its items were modified the entries combined with clinical practice by according to the opinions of expert letter consultation.Results The recovery rate of effective questionnaire in the two rounds of letter inquiries was 94.7%and 100%respectively,and the expert authority coefficient was 0.91,and Kendall harmony coefficient of all indexes ranged from 0.135 to 0.595,and the difference was statistically significant(P<0.01).In the second round of correspondence consultation,the coefficient of variation of each item was 0~0.18,and the full score rate was 38.89%~100%.Finally,a hierarchical management scheme for delirium prevention in ICU patients based on the early delirium prediction model was developed,including 6 first-level indicators,19 second-level indicators,and 50 third-level indicators.Conclusion The hierarchical management of delirium prevention in ICU patients based on the early delirium prediction model is scientific,reliable,targeted,and feasible.It can provide a reference for future ICU delirium prevention and management.
