摘要
目的分析自拟养血除湿汤配合西替利嗪治疗尿毒症维持性血液透析(MHD)伴发皮肤瘙痒血虚夹湿型的疗效,为临床治疗提供参考依据。方法选出该院2019年4月—2020年4月收治的78例尿毒症MHD伴发皮肤瘙痒血虚夹湿型患者,进行随机对照分组,西医组与中西医组各39例,西医组应用西替利嗪治疗,中西医组应用自拟养血除湿汤配合西替利嗪治疗,对比两组患者的症状评分、肾功能指标(血清肌酐、尿素氮、血钙、血磷)、炎性因子水平(白细胞介素-6、白细胞介素-1β、超敏C反应蛋白)、临床疗效、不良反应发生率以及复发率。结果中西医组治疗后的各症状评分均低于西医组,差异有统计学意义(P<0.05);中西医组治疗后血清肌酐、尿素氮、血磷的检测值均低于西医组,血钙检测值高于西医组,差异有统计学意义(P<0.05);中西医组治疗后白细胞介素-6、白细胞介素-1β、超敏C反应蛋白的检测值均低于西医组,差异有统计学意义(P<0.05);中西医组的总有效率高于西医组,差异有统计学意义(P<0.05);中西医组的不良反应发生率与西医组相近,差异无统计学意义(P>0.05);中西医组的复发率低于西医组,差异有统计学意义(P<0.05)。结论自拟养血除湿汤配合西替利嗪治疗尿毒症MHD伴发皮肤瘙痒血虚夹湿型的疗效理想,可明显减轻患者的临床症状,提高生活质量,改善肾功能指标与炎性因子水平,总有效率高,不增加不良反应,且复发率低,值得临床推广与应用。
Objective To analyze the efficacy of Self-made Yangxue Chushi Decoction(自拟养血除湿汤)combined with cetirizine in the treatment of uremic maintenance hemodialysis(MHD)with skin pruritus of blood deficiency and dampness syndrome so as to provide reference basis for clinical treatment.Methods Seventy-eight patients with uremic MHD with skin pruritus,blood deficiency and dampness in our hospital from April 2019 to April 2020 were selected and divided into two groups according to different treatment schemes.There were 39 cases in the western medicine group and the combination group respectively.The western medicine group was treated with cetirizine,and the combination group was treated with Self-made Yangxue Chushi Decoction combined with cetirizine.The symptom scores,renal function indexes(serum creatinine,urea nitrogen,blood calcium,blood phosphorus),inflammatory factor levels(interleukin-6,interleukin-1β),hypersensitivity C-reactive protein,clinical efficacy,adverse reaction rate and recurrence rate of the two groups were compared.Results The scores of symptoms in the combination group were lower than those in the western medicine group(P<0.05).After treatment,the levels of serum creatinine,urea nitrogen and blood phosphorus in the combination group were lower than those in the western medicine group,and the value of blood calcium was higher than that in the western medicine group(P<0.05).The levels of interleukin-6,interleukin-1βand high-sensitivity C-reactive protein after treatment in the combination group were lower than those in western medicine group(P<0.05).The total effective rate of the combination group was significantly higher than that of Western medicine group(P<0.05).The incidence of adverse reactions in the combination group was similar to that in the western medicine group,and the difference was not statistically significant(P>0.05).The recurrence rate of the combination group was significantly lower than that of the western medicine group(P<0.05).Conclusion Self-made Yangxue Chushi Decoc
