摘要
目的探讨腹腔镜下子宫肌瘤剔除术治疗子宫肌瘤的临床效果。方法选取102例子宫肌瘤患者,根据随机数字表法分为对照组与试验组,各51例。试验组给予腹腔镜下子宫肌瘤剔除术治疗,对照组给予开腹子宫肌瘤剔除术治疗。比较两组手术效果。结果试验组手术时间长于对照组,术中出血量少于对照组,住院时间及术后初次排气均短于对照组,差异有统计学意义(P<0.05)。手术前,两组患者应激反应水平比较,差异无统计学意义(P>0.05);术后3 d,试验组醛固酮(ALD)、皮质醇(Cor)及去甲肾上腺素(NE)水平均低于对照组(P<0.05)。手术前,两组患者卵泡刺激素(FSH)、促黄体生成素(LH)水平比较,差异无统计学意义(P>0.05);术后3 d,试验组FSH、LH水平均低于对照组(P<0.05)。手术6个月后,试验组并发症总发生率5.88%低于对照组的23.53%(P<0.05)。结论对子宫肌瘤患者采取腹腔镜下子宫肌瘤剔除术治疗手术时间略长,但手术效果突出,可降低患者应激反应及内分泌功能波动,且并发症少,值得临床推广与使用。
Objective To investigate the clinical effect of laparoscopic myomectomy in the treatment of uterine fibroids.Methods 102 patients with uterine fibroids were selected and divided into control group and experimental group according to random number table method,with 51 cases in each group.The experimental group was treated with laparoscopic myomectomy,and the control group was treated with open myomectomy.The results of operation were compared between the two groups.Results The operation time of experimental group was longer than that of control group,the intraoperative blood loss was less than that of control group,the length of hospital stay and the primary postoperative exhaust were shorter than that of control group,the differences were statistically significant(P<0.05).Before operation,there were no significant differences in stress response level between the two groups(P>0.05).3 days after surgery,the levels of aldosterone(ALD),cortisol(Cor)and norepinephrine(NE)in experimental groups were lower than those in control group(P<0.05).Before operation,there were no significant differences in the levels of follicle stimulating hormone(FSH)and luteinizing hormone(LH)between two groups(P>0.05).On day 3 after surgery,FSH and LH levels in experimental groups were lower than those in control group(P<0.05).After 6 months,the total complication rate of experimental group was 5.88%lower than that of control group(23.53%)(P<0.05).Conclusion Laparoscopic myomectomy for patients with uterine fibroids takes a little longer surgery time,but the surgical effect is outstanding,can reduce the stress response and endocrine function fluctuations of patients,and fewer complications,worthy of clinical promotion and use.
