摘要
目的分析基于加速康复外科(ERAS)理念的综合护理对胸腔镜下肺癌根治术患者围手术期的护理效果。方法选取2021年1月至2022年9月河南省胸科医院收治入院的200例肺癌患者,随机分为ERAS组与常规组,每组100例。所有患者入院后均行胸腔镜下肺癌根治术治疗,常规组患者围手术期接受常规护理措施,ERAS组围手术期接受基于ERAS理念的综合护理措施。比较两组患者术后恢复情况、并发症发生情况,比较两组患者术后疼痛水平,比较两组患者入院时及出院前的心理状态、疾病认知水平及自我管理能力。结果相较于常规组,ERAS组手术时间、气管拔管时间、首次排气时间、引流管留置时间、自主咳嗽时间、下床活动时间及住院时间更短(P<0.05);相较于入院时,两组患者出院前焦虑自评量表(SAS)及抑郁自评量表(SDS)评分均降低,疾病认知问卷(B-IPQ)及癌症患者自我管理效能量表(SUPPH)评分均升高,且ERAS组SAS及SDS评分更低,B-IPQ及SUPPH评分更高(P<0.05);相较于常规组,ERAS组并发症发生率更低(P<0.05)。结论基于ERAS理念的综合护理有助于促进胸腔镜下肺癌根治术患者围手术期患者术后恢复,提高疾病认知水平及自我管理能力,改善患者心理状态,减少并发症的发生。
Objective To analyze the perioperative nursing effect of comprehensive nursing based on the concept of enhanced recovery after surgery(ERAS)on patients undergoing thoracoscopic radical resection of lung cancer.Methods A total of 200 patients with lung cancer admitted from January 2021 to September 2022 in Henan Chest Hospital were randomly divided into ERAS group and conventional group,100 cases with each group.All patients were treated with thoracoscopic radical resection of lung cancer after admission.The patients in the conventional group were treated with routine nursing measures during the perioperative period,while the patients in the ERAS group were treated with comprehensive nursing measures based on the ERAS concept during the perioperative period.The postoperative recovery and complications of the two groups of patients were compared,the postoperative pain level of the two groups of patients was compared,and the psychological state,disease cognition level and self-management ability of the two groups of patients at admission and before discharge were compared.Results Compared with the conventional group,ERAS group had shorter operation time,tracheal extubation time,first exhaust time,drainage tube retention time,spontaneous cough time,ambulation time and hospitalization time(P<0.05).Compared with the patients at admission,the scores of self-rating anxiety scale(SAS)and self-rating depression scale(SDS)before discharge in the two groups were significantly lower,the scores of brief illness perception questionnaire(B-IPQ)and strategies used by people to promote health(SUPPH)were significantly higher,and the SAS and SDS scores in ERAS group were lower,and the B-IPQ and SUPPH scores were higher(P<0.05).The complication rate of ERAS group was lower than that of routine group(P<0.05).Conclusion The implementation of comprehensive nursing based on ERAS concept in the perioperative period of patients undergoing thoracoscopic radical resection of lung cancer is helpful to promote the postoperative recovery of pat
