摘要
目的研究甲状腺癌临床病理特征与前上纵隔淋巴结转移的相关性。方法回顾性分析2020年1月至2022年6月安顺市人民医院收治的80例甲状腺癌患者的临床资料,比较不同病理类型、病理分期、肿瘤个数、肿瘤直径及腺外侵犯等临床病理特征的前上纵隔淋巴结转移发生情况,并进行关联性分析。结果病理类型和腺外侵犯与前上纵隔淋巴结转移关联(x^(2)=15.283,P=0.002;x^(2)=5.635,P=0.018);前上纵隔淋巴结转移与病理分期和肿瘤长径关联(x^(2)=21.042,P<0.05;x^(2)=9.363,P=0.009),与肿瘤个数关联(x^(2)=10.112,P=0.001)。结论甲状腺癌患者出现前上纵隔淋巴结转移情况与其临床病理特征密切相关,恶化程度越高的病理类型和分期发生率越高,单发性肿瘤患者发生率高于多发性患者,肿瘤越大发生率越高,腺外侵犯发生率越高。
Objective To investigate the correlation between clinicopathological features of thyroid cancer and tumor metastasis to the anterior and upper mediastinal lymph nodes.Methods The clinical data of 80 patients with thyroid cancer admitted to our hospital between January 2020 and June 2022 were retrospectively analyzed to compare the occurrence of tumor metastasis to anterior and upper mediastinal lymph nodes accor-ding to clinicopathological features including pathological types and stages,the number of tumors,tumor dia-meter and extrathyroidal invasion.A correlation analysis was performed.Results According to Pearson′s correl-ation analysis,tumor metastasis to anterior and upper mediastinal lymph nodes was in non-linear correlations with pathological types and extrathyroidal invasion(x^(2)=15.283,P=0.002;x^(2)=5.635,P=0.018),and in linear correlations with pathological stage,number of tumors and tumor diameter(P<0.05).The correlations of tumo-rmetastasis to anterior and upper mediastinal lymph nodes was positive with pathological stage and tumor diameter(x^(2)=21.042,P<0.05;x^(2)=9.363,P=0.009)and nega-tive with number of tumors(x^(2)=10.112,P=0.001).Conclusion In patients with thyroid cancer,tumor metastasis to anterior and upper mediastinal lymph nodes closely correlates with their clinicopathological features,and is more likely to occur in patients with more malignant pathological types and advanced stages,in patients with solitary tumor than in those with multiple lesions,in those with larger tumor size,and with extrathyroidal invasion.
