摘要
目的观察益气清热养阴中药方(TCM)对局部晚期非小细胞肺癌(NSCLC)患者免疫功能的影响。方法2019年1月-2020年12月纳入106例NSCLC患者,按随机数字表法分为常规化疗组、联合治疗组,每组各53例,分别给予常规化疗(方案)、常规化疗(方案)+自拟益气清热养阴中药方治疗。连续治疗两个疗程之后,进行影像学检查并比较两组的治疗结果。观察指标包括:疗效[中医学临床证候(咳嗽、痰血、神疲乏力、食欲不振)、肿瘤客观缓解率、疾病控制率]、不良反应(白细胞下降)、体力状况(KPS评分)、血清学免疫指标(T淋巴细胞亚群细胞CD3+、CD4^(+)、CD4^(+)/CD8^(+))。结果两组治疗后体力、中医临床证候均明显改善。常规化疗组的肿瘤客观缓解率(RR)为24.53%(13/53),疾病控制率(OR)为58.49%(31/53),联合治疗组为43.40%(23/53)、81.13%(43/53),两组RR(χ^(2)=0.080)、OR(χ^(2)=0.120)比较,差异均无统计学意义(P>0.05)。常规化疗组与联合治疗组不良反应发生情况比较,差异具有统计学意义(Hc=27.890,P<0.05)。治疗后,与常规化疗组相比,联合治疗组免疫指标T细胞数CD3+,CD4^(+),CD4^(+)/CD8^(+)显著提高(均P<0.05)。结论益气清热养阴中药方治疗NSCLC可稳定肿瘤病灶,改善患者体力、中医学临床证候,并提高免疫水平。
Objective To observe the effect of Yiqi Qingre Yangyin Decoction on immune function of patients with locally advanced nonsmall cell lung cancer(NSCLC).Methods From January 2019 to December 2020,106 patients with NSCLC were included.According to the random number table,they were divided into conventional chemotherapy group and combined treatment group,with 53 patients in each group.They were given conventional chemotherapy(scheme),conventional chemotherapy(scheme)+self-made Chinese medicine for supplementing qi,clearing heat and nourishing yin.After two consecutive courses of treatment,imaging was performed and the treatment results of the two groups were compared.The observation indicators include:curative effect[clinical syndromes of TCM(cough,phlegm and blood,fatigue,loss of appetite),objective remission rate of tumor,disease control rate],adverse reaction(white blood cell decline),physical strength(KPS score),serological immune indicators(T lymphocyte subpopulation cells CD3+,CD4^(+),CD4^(+)/CD8^(+)).Results After treatment,the physical strength and TCM clinical syndromes of the two groups were significantly improved.The objective tumor remission rate(RR)of the conventional chemotherapy group was 24.53%(13/53),the disease control rate(OR)was 58.49%(31/53),and the combined treatment group was 43.40%(23/53)and 81.13%(43/53).The RR of the two groups was(χ^(2)=0.080)、OR(χ^(2)=0.120)and there was no significant difference(P>0.05).There was a statistically significant difference between the conventional chemotherapy group and the combined treatment group in the incidence of adverse reactions and leukopenia(Hc=27.890,P<0.05).Compared with the conventional chemotherapy group,the number of T cells CD3+,CD4^(+),CD4^(+)/CD8^(+)in the combined treatment group were significantly increased(all P<0.05).Conclusion The treatment of NSCLC with Yiqi Qingre Yangyin decoction can stabilize the tumor focus,improve the patient's physical strength,TCM clinical syndromes,and improve the immune level.
