摘要
目的探讨老年COPD患者基于系统评估的“医院-门诊-居家”一体化出院计划的实施效果。方法选取2019年8月—2020年5月宁夏医科大学总医院呼吸与危重症医学科收住的120例COPD患者为研究对象,根据患者基本资料具有可比性的原则分为观察组和对照组,每组60例。对照组给予常规出院指导及随访;观察组在对照组处置基础上进行基于系统评估的“医院-门诊-居家”一体化的出院计划模式护理。比较两组出院准备度、自我效能、出院服务满意度、非计划性再入院、急诊住院情况。结果观察组出院准备度得分、自我效能感得分、出院服务满意度高于对照组,差异有统计学意义(P<0.05);患者急诊入院、非计划性再入院情况低于对照组。差异有统计学意义(P<0.05)。结论基于评估的“医院-门诊-居家”联动模式的出院计划的实施,可完善COPD患者的院外管理,改善患者预后,减少非计划就医和再入院率,为我国延续性护理模式的研究提供依据。
Objective To investigate the effectiveness of the implementation of an integrated"hospital-outpatient-home"discharge plan for elderly patients with COPD based on systematic assessment.Methods 120 patients with COPD admitted to the Department of Respiratory and Critical Care Medicine,General Hospital of Ningxia Medical University from August 2019 to May 2020 were selected for the study.In the control group,the patients were given routine discharge instructions and follow-up;in the observation group,the patients were given an integrated"hospital-outpatient-home"discharge planning model based on systematic assessment based on the treatment of the control group.The two groups were compared in terms of readiness for discharge,self-efficacy,satisfaction with discharge services,unplanned readmissions and emergency admissions.Results The observation group had higher discharge readiness scores,self-efficacy scores and satisfaction with discharge services than the control group,the differences were statistically significant(P<0.05).Patients had lower emergency admissions and unplanned readmissions than the control group.The differences were statistically significant(P<0.05).Conclusion The implementation of a discharge plan based on the evaluation of the"hospital-outpatient-home"linkage model can improve the out-of-hospital management of COPD patients,improve their prognosis,reduce unplanned visits and readmission rates,and provide a basis for the study of continuity nursing models in our country.
