摘要
目的探究芩葶清肺汤联合穴位敷贴治疗痰热郁肺证慢性阻塞性肺疾病急性加重期(AECOPD)患者的临床疗效。方法选取湘潭县中医医院2020年4月至2022年4月收治的65例AECOPD患者为研究对象,按随机数字表分为对照组(n=32)、联合组(n=33),对照组给予穴位敷贴治疗,联合组给予穴位敷贴联合芩葶清肺汤治疗。并比较两组临床疗效、中医证候积分、动脉血气指标、肺功能[呼气峰值流速(PEF)、用力肺活量(FVC)、第1秒用力呼气容积/FVC(FEV1/FVC)]、炎性因子[白介素-6(IL-6)、C反应蛋白(CRP)、降钙素原(PCT)]。结果联合组总有效率93.94%高于对照组71.88%(P<0.05);治疗后联合组喘息、咳嗽、咳痰、便秘中医证候积分均低于对照组(P<0.05);治疗后联合组血氧饱和度(SaO_(2))、氧分压[p(O_(2))]、PEF、FVC、FEV1/FVC均高于对照组,二氧化碳分压[p(CO_(2))]低于对照组(P<0.05);治疗后联合组PCT、CRP、IL-6水平均低于对照组(P<0.05)。结论芩葶清肺汤联合穴位敷贴可提高痰热郁肺证AECOPD患者疗效,改善肺功能、动脉血气指标,并降低炎性因子表达。
Objective To explore the clinical efficacy of Qinting Qingfei decoction combined with acupoint application in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)with phlegm heat stagnation in the lung.Methods 65 patients with AECOPD admitted to Xiangtan County Hospital of Traditional Chinese Medicine from April 2020 to April 2022 were selected as the study subjects.They were divided into control group(n=32)and combined group(n=33)according to the random number table.The control group was treated with acupoint application,and the combined group was treated with acupoint application+Qinting Qingfei decoction.The clinical efficacy,TCM syndrome score,arterial blood gas index,lung function[peak expiratory flow rate(PEF),forced vital capacity(FVC),forced expiratory volume in the first second/FVC(FEV1/FVC)],inflammatory factors[interleukin-6(IL-6),C-reactive protein(CRP),procalcitoni(PCT)]were compared between the two groups.Results The total effective rate of the combined group was 93.94%higher than that of the control group 71.88%(P<0.05);After treatment,the TCM syndrome scores of wheezing,cough,expectoration and constipation in the combined group were lower than those in the control group(P<0.05);After treatment,the blood oxygen saturation(SaO_(2)),partial pressure of oxygen[p(O_(2))],PEF,FVC,and FEV1/FVC in the combined group were higher than those in the control group,and the partial pressure of carbon dioxide[p(CO_(2))]was lower than that in the control group(P<0.05);After treatment,PCT,CRP and IL-6 in the combined group were lower than those in the control group(P<0.05).Conclusion Qinting Qingfei decoction combined with acupoint application can improve the curative effect of AECOPD patients with phlegm-heat stagnation of lung syndrome,improve lung function,arterial blood gas index,and reduce the expression of inflammatory factors.
