摘要
目的:评估玻璃体腔注射雷珠单抗与眼底激光联合治疗重度非增殖期糖尿病性视网膜病变(NPDR)的临床效果。方法:选取2020年6月—2022年5月南京医科大学附属江宁医院眼科收治的66例(132眼)重度NPDR患者作为研究对象,按照治疗方案分为三组。1组22例(44眼)采用532 nm眼底激光全视网膜光凝治疗;2组24例(48眼)采用玻璃体腔注射雷珠单抗治疗;3组20例(40眼)采用玻璃体腔注射雷珠单抗联合激光治疗。对比三组治疗前后最佳矫正视力(BCVA)及眼压,并发症发生情况,2组与3组玻璃体腔注射雷珠单抗次数。结果:治疗前及治疗1、3、6个月三组BCVA及眼压比较,差异无统计学意义(P>0.05)。3组并发症发生率低于1组,差异有统计学意义(P<0.05)。3组玻璃体腔注射雷珠单抗次数少于2组,差异有统计学意义(P=0.000)。结论:玻璃体腔注射雷珠单抗联合眼底激光治疗重度NPDR患者视力的提升更为快速,对提升并保护重度NPDR患者的中期视力有较为显著的临床价值。
Objective:To evaluate the clinical effect of intravitreal injection of ranibizumab and fundus laser in the treatment of severe non-proliferative diabetic retinopathy(NPDR).Methods:A total of 66 patients(132 eyes)with severe NPDR admitted to the Ophthalmology Department of the Affiliated Jiangning Hospital of Nanjing Medical University from June 2020 to May 2022 were selected as the study subjects,and they were divided into three groups according to the treatment plan.Group 1 included 22 patients(44 eyes),who were treated with 532 nm fundus laser panretinal photocoagulation;Group 2 included 24 patients(48 eyes),who were treated with intravitreal injection of ranibizumab;Group 3 included 20 patients(40 eyes),who were treated with intravitreal injection of ranibizumab combined with laser.The best corrected visual acuity(BCVA)and intraocular pressure in the three groups before and after treatment,the occurrence of complications,and the times of intravitreal injection of ranibizumab in Group 2 and Group 3 were compared.Results:There was no statistically significant difference in BCVA and intraocular pressure among the three groups before treatment and after 1 month,3 months and 6 months of treatment(P>0.05).The incidence of complications in Group 3 was lower than that in Group 1,and the difference was statistically significant(P<0.05).The times of intravitreal injection of ranibizumab in Group 3 was less than that in Group 2,and the difference was statistically significant(P=0.000).Conclusion:Intravitreal injection of ranibizumab combined with fundus laser in the treatment of severe NPDR patients improves vision more rapidly,and has significant clinical value for improving and protecting the medium-term vision of patients with severe NPDR.
