摘要
目的探讨Graves病患者血清性激素结合球蛋白(SHBG)与相关生化指标的关系。方法研究纳入大同市第五人民医院2016年1月至2020年12月收治Graves病患者共304例,根据性别分组,分析相关临床指标,采用Spearman检验和多元线性回归模型评价SHBG与相关生化指标间相关性。结果男性和女性Graves病患者SHBG、促黄体生成素(LH)、催乳素(PRL)、雌二醇(E2)、孕酮(P)、睾酮(T)、脱氢表雄酮硫酸盐(DHEAS)、总蛋白、白蛋白(ALB)、前ALB及碱性磷酸酶(ALP)水平比较,差异有统计学意义(P<0.05);Spearman检验结果显示,整体人群SHBG水平均与三碘甲状腺原氨酸(T3)、甲状腺素(T4)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)水平呈正相关(P<0.05);整体人群SHBG水平与ALB、前ALB水平呈负相关(P<0.05);男性患者SHBG水平与甲状腺球蛋白、LH、E2、T及ALP水平呈正相关(P<0.05);女性患者SHBG水平与T、ALT及AST水平呈正相关(P<0.05);女性患者SHBG水平与糖化ALB水平呈负相关(P<0.05);多元线性回归模型中分析结果显示SHBG与BMI、ALB及糖化ALB呈负相关(P<0.05);SHBG与FT3呈正相关(P<0.05);女性患者SHBG与FT3、T呈正相关(P<0.05);SHBG与ALB呈负相关(P<0.05);男性患者SHBG与FT4、E2呈正相关(P<0.05)。结论Graves病患者SHBG水平与多种生化指标相关,其中男性和女性患者均受性激素水平独立影响。
【Objective】To investigate the relationship between serum sex hormone binding globulin(SHBG)and related biochemical indices in patients with Graves'disease.【Methods】A total of 304 patients with Graves'disease were chosen in the period from January 2016 to December 2020 in our hospital.All patients were grouped according to gender.The clinical related indexes were analyzed.Spearman test and multiple linear regression model were used to evaluate the correlation between SHBG and laboratory indexes.【Results】There were significant differences in SHBG,luteinizing hormone(LH),prolactin(PRL),estradiol(E2),progesterone(P),testosterone(T),dehydroepiandrosterone sulfate(DHEAS),total protein,albumin(ALB),pre-ALB and alkaline phosphatase(ALP)between male and female patients with Graves'disease(P<0.05).Spearman test showed that SHBG level was positively correlated with the levels of triiodothyronine(T3),thyroxine(T4),free triiodothyronine(FT3)and free thyroxine(FT4)in total people(P<0.05).The levels of SHBG was negatively correlated with ALB and pre-ALB in total people(P<0.05).The SHBG level was positively correlated with levels of thyroglobulin,LH,E2,T and ALP in male patients(P<0.05).The SHBG level was positively correlated with the levels of T,ALT and AST in female patients(P<0.05).The SHBG level was negatively correlated with the level of glycosylated ALB in female patients(P<0.05).Multiple linear regression analysis showed that SHBG was negatively correlated with BMI,ALB and glycosylated ALB,and SHBG was positively correlated with FT3(P<0.05).SHBG was positively correlated with FT3 and T,and negatively correlated with ALB in female patients(P<0.05).The levels of SHBG was positively correlated with FT4 and E2 in male patients(P<0.05).【Conclusion】The level of SHBG in patients with Graves'disease was correlated with variety of biochemical indicators,and both male and female patients were independently affected by sex hormone levels.
