摘要
目的 探讨对尿道狭窄合并尿道结石患者应用经输尿管镜钬激光术的临床疗效及安全性。方法 选择濮阳市人民医院在2019年1月—2020年12月收治的76例尿道狭窄合并尿道结石患者纳入本次研究,并将其按照治疗方案的不同分成观察组(n=38)与对照组(n=38),观察组采用经输尿管镜钬激光术治疗,对照组采用电切镜冷刀术治疗,比较两组临床疗效、手术时间、碎石时间、术中出血量及住院时间指标等一般指标及尿失禁、尿道瘘及出血等并发症发生情况。结果 观察组治疗有效率(97.4%)明显优于对照组(84.2%)(P<0.05);观察组患者的手术时间、碎石时间、术中出血量及住院时间指标均明显优于对照组(P<0.05);观察组患者中发生1例尿失禁、1例尿道瘘和1例出血,总发生率为7.9%;对照组患者中发生3例尿失禁、1例尿道瘘和3例出血,总发生率为18.4%;观察组患者的并发症发生率明显低于对照组(P<0.05)。结论 应用经输尿管镜钬激光术治疗尿道狭窄合并尿道结石能明显提高临床治疗效果,有效减少手术时间、碎石时间、术中出血量及住院时间,大大降低术后并发症的发生风险,使患者康复时间显著缩短,适合临床应用及推广。
Objective To investigate the clinical efficacy and safety of tranureteroscopic holmium laser in patients with urethral stricture and urethral calculi.Methods Seventy-six patients with urethral stricture complicated with urethral calculi admitted to Puyang People’s Hospital from January 2019 to December 2020 were included in this study, and they were divided into observation group(n=38) and control group(n=38) according to different treatment regimens. The observation group was treated with ureteroscopic holmium laser, and the control group was treated with electrosurgical cold knife. The clinical efficacy, operation time, lithotripsy time, intraoperative blood loss, length of hospital stay and the incidence of urinary incontinence, urethral fistula, bleeding and other complications were compared between the two groups.Results The effective rate of observation group was 97.4%, which was significantly better than that of control group(84.2%)(P<0.05). Operation time, lithotripsy time, intraoperative blood loss and length of stay in the observation group were significantly better than those in the control group(P<0.05). In the observation group, there was 1 case of urinary incontinence, 1 case of urethral fistula and 1 case of bleeding, with a total incidence of 7.9%. In the control group, there were 3 cases of urinary incontinence, 1 case of urethral fistula and 3 cases of bleeding, with a total incidence of 18.4%. The incidence of complications in observation group was significantly lower than that in control group(P<0.05).Conclusion The application of tranureteroscopic holmium laser in the treatment of urethral stricture combined with urethral calculi can significantly improve the clinical treatment effect, effectively reduce the operation time, lithotripsy time, intraoperative blood loss and hospitalization time, shorten the recovery time of patients, and reduce the risk of postoperative complications, which is worthy of clinical promotion and application.
