摘要
目的 研究退行性心脏瓣膜病患者血清sKL、Fetuin-A与心脏瓣膜钙化及血管钙化的关系,为临床退行性心脏瓣膜病诊治提供依据。方法 选取某院2016年5月13日-2021年1月13日收治的112例退行性心脏瓣膜病患者作为实验组,同时随机选取110例同期健康体检者作为对照组,实验组所有患者入院后均接受常规治疗,并使用心脏彩超以及腹部X线检查对患者心脏瓣膜钙化及血管钙化进行诊断,同时检测两组入选者血清sKL、Fetuin-A;检测心脏瓣膜钙化和血管钙化患者血清sKL、Fetuin-A;绘制ROC曲线分析血清sKL、Fetuin-A对于患者心脏瓣膜钙化及血管钙化的诊断价值;分析血清sKL、Fetuin-A与患者心脏瓣膜钙化及血管钙化程度的相关性。结果 相比于对照组,实验组血清sKL、Fetuin-A显著更低t_(血清sKL)=11.896,t_(血清Fetuin-A)=26.264;经过心脏彩超检查发现,112例患者有108例患者存在瓣膜钙化.存在心脏瓣膜钙化的患者血清sKL、Fetuin-A水平均显著低于不存在心脏瓣膜钙化的患者t_(血清s KL)=5.594,t_(血清Fetuin-A)=5.338;经过心脏彩超检查发现,112例患者有108例患者存在血管钙化.存在血管钙化的患者血清sKL、Fetuin-A水平均显著低于不存在血管钙化的患者t_(血清sKL)=5.017,t_(血清Fetuin-A)=3.660;经过ROC曲线分析得出:血清sKL、Fetuin-A对于退行性心脏瓣膜病患者发生心脏瓣膜钙化及血管钙化的诊断灵敏度和特异度均较高;经过相关性分析得出血清sKL和Fetuin-A与患者心脏瓣膜钙化和血管钙化程度呈负相关。(P<0.05)。结论 血清sKL、Fetuin-A在退行性心脏瓣膜病发生心脏瓣膜钙化以及血管钙化中发挥着重要的作用,对于患者发生心脏瓣膜钙化以及血管钙化的诊断价值较高。
Objectives This study aims to study the relationship between serum soluble Klotho (sKL) protein,Fetuin-A,and heart valve calcification and vascular calcification in patients with degenerative heart valve disease,so as to provide a basis for clinical diagnosis and treatment of degenerative heart valve disease.Methods A total of112 patients with degenerative heart valve disease admitted to a hospital from May 13,2016 to January 13,2021were selected as the experimental group,and 110 healthy subjects were randomly selected as the control group.All patients in the experimental group received conventional treatment after admission and cardiac color Doppler ultrasound and abdominal X-ray were used to diagnose heart valve calcification and vascular calcification.The serum sKL and Fetuin-A of the selected two groups were examined at the same time.Serum sKL and Fetuin-A in patients with cardiac valve calcification and vascular calcification were examined in all selected individuals.Receiver Operating Characteristic (ROC) curve was plotted to analyze the diagnostic efficacy of serum sKL and Fetuin-A for patients with heart valve calcification and vascular calcification,and the correlation of serum sKL and Fetuin-A with heart valve calcification and vascular calcification was discussed.Results Compared with the control group,the serum sKL and Fetuin-A of the experimental group were significantly lower (t_(serum sKL)=11.896,t_(serum Fetuin-A)=26.264).Cardiac ultrasound revealed valve calcification in 108 of 112 patients.Serum sKL and Fetuin-A levels were significantly lower in patients with heart valve calcification than in those without heart valve calcification(t_(serum sKL)=5.594,t_(serum Fetuin-A)=5.338).Cardiac ultrasound revealed vascular calcification in 108 of 112 patients.Serum sKL and Fetuin-A levels were significantly lower in patients with vascular calcification than in those without vascular calcification (tserum sKL=5.017,tserum Fetuin-A=3.660).ROC curve analysis showed that the sensitivity and specificity of se
