外周血脑源性神经营养因子与阿尔茨海默病关联性的Meta分析 被引量:2

Meta-analysis of the association between brain-derived neurotrophic factor in peripheral blood and Alzheimer's disease

摘要 目的定量考查和评价外周血脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)与阿尔茨海默病(Alzheimer's disease,AD)的关联性。方法全面检索Pubmed、Cochrane library、Web of science、Embase、中国知网、中国生物医学文献数据库、维普全文数据库和万方数据库,搜集与阿尔茨海默病痴呆(dementia with Alzheimer’s type,DAT)和轻度认知障碍(mild cognitive impairment,MCI)患者外周血BDNF相关的病例对照研究,由2名研究者独立按照纳入和排除标准提取资料并进行文献质量评价,采用RevMan Manager 5.3和CMA 3.0进行数据分析。结果共51篇文献纳入分析,总样本7182例,其中DAT组2673例,MCI组1506例,对照组3003例。Meta分析结果显示,DAT患者外周血BDNF水平显著低于正常对照组(SMD=-0.71,95%CI:-0.99~-0.43,P<0.001)(n=5111),MCI组与对照组以及DAT组与MCI组外周血BDNF水平差异无统计学意义。亚组分析结果显示,DAT患者血清BDNF(SMD=-0.85,95%CI:-1.15~-0.55,P<0.001)(n=4425)和MCI患者血清BDNF(SMD=-0.38,95%CI:-0.62~-0.14,P=0.002)(n=2476)水平显著低于正常对照组,且DAT患者血清BDNF(SMD=-0.76,95%CI:-1.37~-0.16,P=0.01)(n=1630)水平低于MCI患者;血浆BDNF水平在DAT、MCI和对照组间差异无统计学意义(P>0.05)。结论DAT和MCI患者血清BDNF水平明显降低,提示血清BDNF水平或许可以成为AD早期诊断的潜在生物标志物。 Objective To evaluate association of peripheral blood brain-derived neurotrophic factor(BDNF)with Alzheimer's disease(AD).Methods Databases including Pubmed,Cochrane library,Web of science,Embase,China National Knowledge Infrastructure,CBM disc,VIP-CSTJ and Wanfang Data were used to collect case-control studies related to the concentration of BDNF in peripheral blood of dementia patients with Alzheimer's type(DAT)and mild cognitive impairment(MCI).After extracting data and appraising the quality of the included studies,meta-analysis were conducted using Review Manager 5.3 and CMA 3.0.Results A total of 51 articles were included in the analysis,with a total subjects of 7182,including 2673 subjects in DAT group,1506 subjects in MCI group,and 3003 subjects in control group.The Meta-analysis showed that the levels of peripheral blood BDNF in patients with DAT were significantly lower than normal control group(SMD=-0.71,95%CI:-0.99--0.43,P<0.001)(n=5111),and there were no statistical differences in peripheral blood BDNF levels between MCI group and control group and between DAT group and MCI group.The subgroup analysis showed that the level of serum BDNF in patients with DAT(SMD=-0.85,95%CI:-1.15--0.55,P<0.001)(n=4425)and MCI(SMD=-0.38,95%CI:-0.62--0.14,P=0.002)(n=2476)was significantly lower than that in normal control group,and the level of serum BDNF(SMD=-0.76,95%CI:-1.37--0.16,P=0.01)(n=1630)in patients with DAT was lower than that in MCI;However,there were no statistical difference among DAT,MCI and control groups in the level of plasma BDNF(P>0.05).Conclusion The patients with DAT and mild cognitive impairment have lower level of serum BDNF,which suggesting that serum BDNF level may be a potential biomarker for early diagnosis of AD.
