摘要
目的 探讨减轻体质量对肥胖的多囊卵巢综合征(PCOS)患者体外受精-胚胎移植(IVF)周期卵巢反应性及妊娠结局的应用。方法 回顾性分析嘉兴市妇幼保健院2015年7月—2021年6月行IVF助孕的178例体质指数(BMI)≥28 kg/m^(2)的肥胖型PCOS患者的资料,减轻体质量5%及以上为减重组,共87例,比较减重组与对照组促排卵卵巢反应性和临床妊娠率等有无差异。结果 减重组的基础体质量和基础BMI显著高于对照组,减重后腺促性腺素(Gn)天数、Gn总量显著低于对照组,D3优质胚胎数高于对照组,差异有统计学意义(P<0.05),减重组胚胎着床率为46.25%,对照组为31.25%,差异有统计学意义(P<0.05)。与对照组相比,减重组临床妊娠率和持续妊娠率有增高的趋势,但差异无统计学意义(P>0.05)。结论 肥胖的PCOS患者减轻体质量后行IVF,卵巢反应性增加,D3优质胚胎数增加,胚胎着床率升高,建议肥胖型PCOS患者IVF助孕前减轻体质量。
Objective To assesse the effect of weight loss on ovarian response and clinical outcome in obese patients with polycystic ovary syndrome(PCOS) undergoing IVF treatment.Methods The data of 178 obese PCOS patients with body mass index(BMI) ≥28 kg/m~2 who underwent IVF from July 2015 to June 2021 were analyzed retrospectively.In total, the weight loss group included 87 patients with a reduction in body weight reaching at least 5%.The ovarian response and clinical outcomes were compared between the weight loss group and the control group.Results The basal body weight and BMI were significantly higher in the weight loss group than in the control group.The days of stimulation and total Gn dose were lower and the number of D3 high-quality embryos was higher in the weight loss group compared with the control group.The implantation rate in the weight loss group and control group were 46.25% and 31.25% respectively(P<0.05).The clinical pregnancy rate and the on-going pregnancy rate were higher in the weight loss group than in the control group, but there was no significant difference.Conclusion Weight loss for obese PCOS women prior to IVF improves ovarian response and resultes in a significantly higher number of D3 high-quality embryos and higher implantation rate.The data suggest that obese PCOS patients reduce weight before IVF.
