摘要
目的以收集的老年慢性胆固醇结石性胆囊炎急性发作为研究对象,中医证型均为肝胆湿热证,观察四逆散合复方茵陈蒿汤加减治疗效果,为临床指导治疗提供重要参考依据。方法选取2020年1月到2022年5月在开封市第二中医院进行治疗的老年慢性胆固醇结石性胆囊炎患者作为研究对象,经纳入标准、排除标准以及样本量估算后,最终纳入100例患者。分组方法选取随机数字表法,对照组、观察组,均50例。对照组患者采用常规对症治疗,观察组在其基础上增加四逆散合复方茵陈蒿汤加减治疗。两组患者均连续干预5d。统计两组综合疗效、实验室相关指标如NF-κB、CCK及CRP、TNF-α水平。结果治疗5 d后,观察组的综合疗效明显优于对照组(P<0.05)。两组患者治疗5 d后的血清NF-κB、CCK及CRP、TNF-α水平均明显降低,且观察组明显低于对照组(P<0.05)。结论对于老年慢性胆固醇结石性胆囊炎急性发作患者,在常规对症治疗的基础上,再增加四逆散合复方茵陈蒿汤加减治疗,可以有效提高综合疗效,此种治疗作用可能与其改善NF-κB调控、抑制炎症等相关机制,调控胆汁的分泌及排泄过程有关,具有应用到医学临床的价值,值得去推广应用。
Objective To investigate the acute incidence of chronic cholesterol calculous cholecystitis in the elderly,and to observe the therapeutic effect of Sini SAN combined with compound Yinchenhao decoction,and to provide an important reference for clinical treatment.Method s Elderly patients with chronic cholesterol calculous cholecystitis treated in the Second Middle School Hospital of Kaifeng City from January 2020 to May 2022 were selected as research objects.After the inclusion criteria,exclusion criteria and sample size estimation,100 patients were finally included.Grouping methods Random number table method was selected,control group,observation group,50 cases.The control group was treated with conventional symptomatic treatment,and the observation group was additionally treated with Sini SAN combined with compound Yinchenhao decoction.Both groups were continuously treated for 5 days.Comprehensive efficacy,laboratory-related indexes such as NF-κB,CCK,CRP and TNF-αlevels were counted in the two groups.Results After 5 days of treatment,the comprehensive efficacy of the observation group was significantly better than that of the control group(P<0.05).The levels of NF-κB,CCK,CRP and TNF-αin 2 groups were significantly decreased after 5 days of treatment,and the observation group was significantly lower than the control group(P<0.05).Conclusion For elderly patients with acute onset of chronic cholesterol calculous cholecystitis,on the basis of conventional symptomatic treatment,adding Sini SAN combined with compound Yinchenhao decoction plus or less treatment can effectively improve the comprehensive curative effect.This therapeutic effect may be related to improving the regulation of NF-κB,inhibiting inflammation and other related mechanisms,regulating the secretion and excretion of bile.It has the value of application to medical clinic and is worth popularizing and applying.
