摘要
目的探讨miR-20a-5p、miR-29c-3p在脑卒中后认知功能障碍中的表达水平以及相关影响因素。方法选取河南大学第一附属医院2020-01—2022-06收入的脑卒中患者159例,根据其认知功能情况分为正常组(62例)、轻度障碍组(38例)、中度障碍组(36例)、重度障碍组(23例),测4组患者血清miR-20a-5p、miR-29c-3p水平,分析miR-20a-5p、miR-29c-3p与脑卒中后认知功能障碍关系,以miR-20a-5p、miR-29c-3p鉴别轻度、中度认知功能障碍患者界值(0.51、0.78)为分界点将97例脑卒中后认知功能障碍患者分为miR-20a-5p高表达组、低表达组与miR-29c-3p高表达组、低表达组。采用Logistic回归模型探究miR-20a-5p、miR-29c-3p高、低表达的影响因素。结果正常组miR-20a-5pmRNA表达水平<轻度障碍组<中度障碍组<重度障碍组,正常组miR-29c-3p mRNA表达水平>轻度障碍组>中度障碍组>重度障碍组,差异有统计学意义(P<0.05)。miR-20a-5p高表达组年龄≥68、合并高血压与糖尿病、MMSE得分<15占比显著高于低表达组,差异有统计学意义(P<0.05);miR-29c-3p低表达组年龄≥68、合并高血压与糖尿病、MMSE得分<15占比显著高于高表达组,差异有统计学意义(P<0.05)。Logistic回归分析显示年龄、高血压、糖尿病、MMSE得分是miR-20a-5p、miR-29c-3p表达的影响因素(P<0.05)。结论miR-20a-5p mRNA异常高表达、miR-29c-3p mRNA异常低表达会增加脑卒中后患者认知功能障碍风险,且认知功能障碍越严重,异常表达越明显。年龄、高血压、糖尿病、MMSE得分是miR-20a-5p、miR-29c-3p表达的影响因素。
Objective To analyze the expression level of miR-20a-5p and miR-29c-3p in cognitive dysfunction after stroke and related influencing factors.Methods 159 stroke patients from the First Affiliated Hospital of Henan University from January 2020 to June 2022 were selected and divided into normal group(62 cases),mild disorder group(38 cases),moderate disorder group(36 cases)and severe disorder group(23 cases)according to their cognitive function.The serum miR-20a-5p,miR-29c-3p levels in four groups were measured,and the correlation between miR-20a-5p,miR-29c-3p and cognitive dysfunction after stroke was analyzed,The cut-off value of miR-20a-5p and miR-29c-3p to identify mild and moderate patients with cognitive impairment after stroke was obtained through the subject work curve(ROC).97 patients with cognitive impairment after stroke were divided into high expression group,low expression group,high expression group and low expression group of miR-20a-5p.Logistic regression model was used to explore the influencing factors of high and low expression of miR-20a-5p and miR-29c-3p.Results The expression level of miR-20a-5p in the normal group was less than that in the mild disorder group,less than that in the moderate disorder group,and less than that in the severe disorder group.The expression level of miR-29c-3p in the normal group was more than that in the mild disorder group,more than that in the moderate disorder group,and more than that in the severe disorder group.The difference was statistically significant(P<0.05).The age≥68,hypertension,diabetes and MMSE score<15 in the miR-20a-5p overexpression group were significantly higher than those in the low expression group(P<0.05);The low expression group of miR-29c-3p with age≥68,hypertension,diabetes and MMSE score<15 accounted for significantly higher than the high expression group(P<0.05).Logistic regression analysis showed that age,hypertension,diabetes and MMSE scores were the influencing factors of miR-20a-5p and miR-29c-3p expression(P<0.05).Conclusion The ab
