头穴丛刺联合认知训练治疗脑卒中后认知障碍疗效观察 被引量:17

摘要 目的观察头穴丛刺联合认知训练治疗脑卒中后认知功能障碍(CFI)的疗效。方法共选取脑卒中后CFI患者60例,采用随机数字表法将其分为治疗组及对照组。两组患者均常规给予吡拉西坦口服及一般康复、作业、认知疗法治疗;治疗组在对照组治疗方法基础上给予头穴丛刺治疗。于治疗前、治疗4星期后采用简明精神状态检查量表(MMSE)对两组患者CFI程度进行评定,同时采集患者治疗前、治疗后晨血测定血浆皮质醇水平变化情况。结果治疗前两组患者MMSE评分及血浆皮质醇水平组间差异均无统计学意义(P>0.05);经4星期治疗后,两组患者MMSE评分及血浆皮质醇水平均较治疗前明显改善(P<0.05);并且治疗组的改善幅度相对较显著(P<0.01)。结论头穴丛刺联合认知训练可进一步改善脑卒中患者CFI程度,对提高患者日常生活活动能力具有改善作用,改善卒中后患者生活质量。 Objective To observe the clinical effect of cluster needling at scalp acupoints plus cognition training in treating post-stroke cognitive function impairment (CFI). Method Sixty patients with post-stroke CFI were enrolled and randomized into a treatment group and a control group according to random number table. Patients in the two groups all took Piracetam, and received basic rehabilitation, occupational, and cognition therapies. In addition, the treatment group also received cluster needling at scalp acupoints. Mini-Mental State Examination (MMSE) was adopted in evaluating the severity of CFI before treatment and after 4-week treatment. Meanwhile, morning plasma cortisol level was measured before and after treatment. Result Before treatment, the differences in MMSE score and plasma cortisol level between the two groups were statistically insignificant (P〉0.05); after 4-week treatment, the two groups both had significant improvements in MMSE score and plasma cortisol level (P^0.05), and the improvements in the treatment group were markedly superior to those in the control group (P〈0.05). Conclusion Cluster needling at scalp acupoints plus cognition training can improve the CFI level, activities of daily living, and living quality of the post-stroke patients.
