摘要
目的 研究两种不同针刀入路治疗(lumbar disc herniation LDH)的临床疗效差异。方法 选取符合纳入标准的70例LDH患者,使用随机数字表法分为痛点入路组(35例,脱落1例)与椎间孔点入路组(35例,脱落2例),分别采用痛点入路和椎间孔点入路针刀松解治疗。在治疗前、治疗后、随访3个月时,分别采用VAS评分和JOA评分评定疼痛及功能,并在治疗后、随访3个月时评估疗效。结果 治疗后,痛点组VAS评分显著优于椎间孔点组(P<0.05);两组JOA评分及疗效差异无统计学意义(P>0.05);随访3个月时,痛点组VAS、JOA评分及疗效均显著优于椎间孔点组(P<0.05)。结论 两者短期疗效总体相当,但痛点入路在改善疼痛方面效果明显,且在远期疗效上更有优势。
Objective To explore the clinical efficacy of two different needle-knife approaches in the treatment of lumbar disc herniation(LDH). Methods A total of 70 patients with LDH who met the inclusion criteria were randomly divided into pain point approach group(35 cases, 1 case of abscission) and intervertebral foramen approach group(35 cases, 2 cases of abscission). They were treated with needle-knife release through pain point approach and intervertebral foramen approach respectively. The pain and function were evaluated by visual analogue scale(VAS) and JOA score before treatment, after treatment and 3-month follow-up, and the curative effect was evaluated after treatment and at the 3-month follow-up. Results After treatment, the VAS score of pain point group was better than that of intervertebral foramen point group, and the difference was statistically significant(P<0.05). There were no significant differences in JOA score and curative effect between the two groups(P>0.05). During the 3-month follow-up, the VAS, JOA scores and curative effect of the pain point group were better than those of the intervertebral foramen point group, and the differences were statistically significant(P<0.05). Conclusion The short-term curative effects of the two methods are generally the same, but the pain point approach is effective in improving pain, and has more advantages in long-term curative effect.
