摘要
目的探讨光学相干断层成像术(OCT)检查在屈光参差性弱视患儿诊断中的应用。方法选取郑州大学附属儿童医院2019年10月—2020年1月屈光参差性弱视患儿45例,90眼,依据视力情况分为屈光参差性弱视组即观察组(45眼)、对侧非屈光参差性弱视组即对照组(45眼)。两组均采用OCT扫描检查患儿双眼3 mm×3 mm及6 mm×6 mm黄斑区行、4.5 mm×4.5 mm视盘,比较两组黄斑各区厚度[中央区、上方(3 mm)、下方(3 mm)、鼻侧(3 mm)、颞侧(3 mm)]、视盘周围各区域视神经纤维层(RNFL)厚度(上方、下方、鼻侧、颞侧、平均)、黄斑区神经节细胞复合体(GCC)各区域厚度(上方、下方、平均)。结果OCT对两组视网膜视盘和视网膜黄斑厚度、视盘周围视神经纤维层厚度及黄斑区神经节细胞复合体扫描显示,观察组黄斑中央区、上方(3 mm)、下方(3 mm)、鼻侧(3 mm)及颞侧(3 mm)厚度均大于对照组(P<0.05);观察组视盘周围上方、下方、鼻侧、颞侧及平均RNFL厚度均大于对照组(P<0.05);观察组黄斑区GCC上方、下方、平均厚度均小于对照组(P<0.05)。结论OCT检查应用于屈光参差性弱视患儿具有较高诊断价值,可提高诊断灵敏度、准确度,为临床病情诊断提供有效依据,具有临床推广价值。
Objective To investigate the application of optical coherence tomography(OCT)in the diagnosis of anisometropia amblyopia.Methods A total of 45 children with anisometropia amblyopia(90 eyes)were selected from Our hospital from October 2019 to January 2020,and divided into anisometropia amblyopia group(observation group,45 eyes)and contralateral non-anisometropia amblyopia group(control group,45 eyes)according to visual condition.OCT scanning was used in both groups to examine the 3 mm×3 mm,6 mm×6 mm macular area and 4.5 mm×4.5 mm optic disc.Compare two groups of macular thickness districts[central and above(3 mm),below(3 mm),the nasal side(3 mm),temporal side(3 mm)],various regions around the DVD optic nerve fiber layer(RNFL)thickness(above and below,the nasal side,temporal side,on average),macular area ganglion cell complex(GCC)regional thickness(above,below,on average).Results OCT scans of retinal disc and macular thickness,optic nerve fiber layer thickness around optic disc and ganglion cell complex in macular area of the two groups showed that,the thickness of central macula,upper macula(3 mm),lower macula(3 mm),nasal macula(3 mm)and temporal macula(3 mm)in the observation group were higher than those in the control group(P<0.05).The average RNFL thickness in the observation group was higher than that in the control group(P<0.05).The upper,lower and average thickness of GCC in macular area of observation group were lower than those in control group(P<0.05).Conclusion OCT has high diagnostic value in children with anisometropia amblyopia,can improve diagnostic sensitivity and accuracy,provide effective basis for clinical diagnosis,and has clinical promotion value.
