基于数据挖掘的王成祥治疗肺间质纤维化用药规律研究 被引量:1

摘要 目的基于数据挖掘方法探索并总结王成祥教授治疗肺间质纤维化用药规律。方法收集王成祥教授2020年9月2日-2021年11月10日于北京中医药大学第三附属医院专家诊室诊治的肺间质纤维化患者病案,经筛选后构建病案数据库,利用SPSS Statistics 20.0软件对用药频次进行统计分析,并对所用药物的性味归经进行统计,使用SPSS Modeler 18.0中的Apriori算法对药物的关联规则进行分析,对频次排名前25位的高频药物进行系统聚类分析,挖掘其核心药物组成。结果共筛选出203首处方,涉及119味中药,总频次3537次,其中频次前10位的药物分别为清半夏、炙甘草、五味子、党参、茯苓、苦杏仁、干姜、黄芩、枇杷叶、麦冬;药物药性以温、寒、平为主,药味以辛、甘、苦为主,归经主入肺经。常用药物组合为麦冬-五味子、麦冬-党参-五味子、陈皮-清半夏,通过聚类分析得到8组核心药物。结论王成祥教授治疗肺间质纤维化采用清热理气化痰治标,益气养阴润肺治本的治疗大法,并辅以活血药祛瘀通络、虫类药搜风止痉。 Objective To explore and summarize the prescription and medication law of Professor Wang Chengxiang in the treatment of pulmonary interstitial fibrosis based on data mining.Methods The cases of patients with pulmonary interstitial fibrosis diagnosed and treated by Professor Wang Chengxiang in the expert consulting room of Beijing University of Chinese Medicine Third Affiliated Hospital from September 2,2020 to November 10,2021 were collected.After screening,the medical record database was constructed.The medication frequency was statistically analyzed by SPSS statistics 20.0 software,and statistics were made on the properties,tastes and meridians of medication;the Apriori algorithm in SPSS modeler 18.0 was used to statistically analyze the association rules of drugs;the top 25 high-frequency drugs were analyzed by systematic cluster analysis to mine their core drug composition.Results Finally,203 prescriptions were screened out,involving 119 traditional Chinese medicines,with a total frequency of 3537 times.The top 10 types of Chinese materia medica were Pinellia ternata,roasted licorice,Schisandra chinensis,Codonopsis pilosula,Poria cocos,bitter almond,dried ginger,Scutellaria baicalensis,loquat leaves,and Ophiopogon japonicus;the drug properties were mainly warm,cold and flat,and the drug tastes were mainly pungent,sweet and bitter;the meridians mainly enter the lung meridians.The common medicinal combinations were Ophiopogon japonicus-Schisandra chinensis,Ophiopogon japonicus-Codonopsis pilosula-Schisandra chinensis,tangerine peel-Pinellia ternata.8 groups of core drugs were obtained by cluster analysis.Conclusion Professor Wang Chengxiang’s treatment of pulmonary interstitial fibrosis adopts the treatment method of clearing heat,regulating qi,resolving phlegm to treat symptoms,nourishing qi,nourishing yin and moistening the lung to treat the root cause,and supplemented with blood-activating drugs to remove blood stasis and dredge meridians,insectdrugs to dissipate wind and stop spasm.
