摘要
中医药在结直肠癌防治过程发挥作用的3个主要阶段包括癌前病变期、协同治疗期、临床康复期。癌毒病机理论认为,“虚、毒、湿、热、瘀”是结直肠癌的主要病理因素。脾虚毒蕴是结直肠癌发生发展的核心病机,脾气亏虚是发病之本,癌毒内蕴是病机关键。癌前病变期的基本病机为“湿热蕴结、脾气亏虚”,协同治疗期的基本病机为“湿热瘀毒、脾气亏虚”,临床康复期的基本病机为“脾肾两虚、湿热瘀毒”。针对结直肠癌3个主要阶段的病机及其证候特点,制定相应治则治法,构建基于癌毒病机理论的结直肠癌全程防治体系,对指导中医药在防治结直肠癌的全过程中发挥作用具有重要意义。
There are three major stages in the prevention and treatment of colorectal cancer for traditional Chinese medicine(TCM),including precancerous lesion period,collaborative treatment period,clinical rehabilitation period. The pathogenesis theory of cancer toxin reveals that ‘deficiency,toxin,dampness,heat,stasis’ are the main pathological factors of colorectal cancer. The core pathogenesis of colorectal cancer is ‘spleen deficiency and toxin accumulation’ in which spleen deficiency is the basis of the disease and toxin accumulation is the key pathogenesis. The basic pathogenesis of precancerous lesion period is ‘dampness-heat accumulation,deficiency of spleen qi’;the basic pathogenesis of collaborative treatment period is ‘dampness-heat and stagnant toxin,deficiency of spleen qi’;the basic pathogenesis of clinical rehabilitation period is ‘spleenkidney deficiency,dampness-heat and stagnant toxin’. According to the pathogenesis characteristics of colorectal cancer at different stages,it is of great significance to formulate the corresponding treatment principles and treatment methods and construct a prevention and treatment system of colorectal cancer based on the pathogenesis theory of cancer-toxin in a bid to guide TCM in whole process of prevention and treatment of colorectal cancer.
