血糖控制对1型糖尿病儿童生长发育的影响及与胰岛素样生长因子结合蛋白3型的相关性研究 被引量:2

摘要 目的 评估血糖控制对1型糖尿病(T1DM)儿童生长发育的影响,并分析其与胰岛素样生长因子结合蛋白3型(IGFBP-3)的相关性。方法 将2018年7月至2021年12月唐山市妇幼保健院儿科门诊收治的70例青春期前T1DM儿童纳入本次横断面研究,其中35例男性和35例女性,分别纳入研究组1组和2组。另选同时间段内30名年龄匹配的健康儿童纳入对照组,包括15名男性和15名女性。比较3组的儿童身高、体重、身高和体重百分位数、糖化血红蛋白(HbA1c)和IGFBP-3水平,并采用Spearman分析HbA1c与身高、身高百分位数、体重、体重百分位数进行的相关性,IGFBP-3与身高、身高百分位数、体重、体重百分位数、空腹血糖(FPG)、餐后2 h血糖(2 hPBG)、HbA1c的相关性。结果 研究1组、研究2组和对照组的平均身高分别为(129.287±13.41)、(127.73±10.15)、(136.760±13.431) cm,平均体重分别为(31.10±9.45)、(28.33±7.76)、(35.86±9.02) kg,与对照组相比,研究1组、研究2组儿童的身高和体重均明显降低,差异均有统计学意义(P<0.05)。研究1组、研究2组和对照组身高百分位数的为24.58%、28.05%和87.20%,体重百分位数的为42.77%、38.44%和86.46%,与对照组相比,1型糖尿病儿童的身高和体重百分位数均明显降低,差异均有统计学意义(P<0.05)。研究1组、研究2组和对照组的血清IGFBP-3分别为(3978.19±812.50)、(3796.24±795.25)、(4948.75±982.42) ng/mL,研究1组、研究2组儿童的血清IGFBP-3明显低于对照组,差异均有统计学意义(P<0.05)。研究1组、研究2组和对照组的HbA1c分别为(9.71±1.52)%、(9.20±1.17)%和(5.02±0.27)%,与对照组相比,研究1组、研究2组儿童的HbA1c明显增加,差异均有统计学意义(P<0.05)。研究1组与研究2组间上述各指标差异均无统计学意义(P>0.05)。Spearman分析显示,HbA1c与血清IGFBP-3、身高和身高百分位数呈负相关(r=-0.332、-0.269、-0.402),血清IGFBP-3与身高、身高百分位� Objective To evaluate the effect of glycemic control on the growth and insulin growth factor binding protein-3(IGFBP-3), represented by height, weight, height and weight percentiles and for age in a sample of children with type 1 diabetes mellitus(T1DM). Methods Seventy prepubertal children with T1DM admitted to Tangshan Maternal and Child Health Hospital from July 2018 to December 2021 were enrolled in this cross-sectional study, including 35 males and 35 females(inclusion in study group 1 and 2) and 30 healthy children age matched at the same periods were included in the control group, including 15 males and 15 females. The indexes of the three groups were compaered as follows: anthropometric measures including height, weight and height and weight percentiles, hemoglobin A1c(HbA1c) as well as IGFBP-3 levels. Spearman were used to analyze the correlation between HbA1c and height, height percentile, weight, weight percentile, and the correlation between IGFBP-3 and height, height percentile, weight, weight percentile, fasting plasma glucose(FPG),2 h postprandial blood glucose(2 h PBG), HbA1c. Results The mean height values in the study group 1, the study group 2 and in the control group were(129.287±13.41),(127.73±10.15),(136.760±13.431) cm, respectively, the mean weight values were(31.10±9.45),(28.33±7.76),(35.86±9.02) kg, respectively, the height and weight of children in study group 1 and study group 2 were significantly lower than those in the control group, the differences were statistically significant(P<0.05). The height percentile with median(range) values in the study group 1, the study group 2 and the control group were 24.58%, 28.05%, 87.20%, respectively, and the weight percentile values were 42.77%, 38.44%, 86.46%, respectively, the height and weight percentiles in study group 1 and study group 2 were significantly lower than those in the control group, the differences were statistically significant(P<0.05).
