矮小症儿童骨密度与血清维生素D IGF-1 IGFBP-3的相关性分析 被引量:27

Correlation Analysis Between Bone Mineral Density and Serum Vitamin D IGF-1 and IGFBP-3 in Children with Dwarfism

摘要 目的:分析矮小症儿童骨密度与血清维生素D、胰岛素样生长因子-1(Insulin like growth factor-1,IGF-1)、胰岛素生长因子结合蛋白-3(Insulin growth factor binding protein-3,IGFBP-3)的相关性。方法:选取我院2016年3月至2021年3月收治的140例矮小症儿童,按照其2种药物激发试验后生长激素(Growth hormone,GH)水平,将患儿分别纳入特发性矮小症(Idiopathic short stature,ISS)组、生长激素缺乏(Growth hormone deficiency,GHD)组,并选取同期40名健康体检儿童,纳入对照组。检测各组儿童骨密度及血清维生素D、IGF-1、IGFBP-3水平,并使用Pearson相关性分析,计算骨密度与矮小症儿童血清指标的相关性。结果:对照组、ISS组、GHD组儿童骨密度依次下降,组间比较差异有统计学意义(P<0.05)。对照组、ISS组、GHD组儿童血清25-羟维生素D、IGF-1、IGFBP-3依次下降,组间比较差异有统计学意义(P<0.05)。Pearson相关性分析示,ISS、GHD患儿骨密度与血清25-羟维生素D、IGF-1、IGFBP-3均呈正相关(r=0.627,0.660,0.625,P<0.05)。结论:不同病因所致矮小症患儿均存在不同程度的骨密度下降表现,且患儿骨密度下降幅度与血清25羟-维生素D、IGF-1、IGFBP-3下降幅度密切相关。 Objective:To analyze the correlation between bone mineral density and serum vitamin D,insulin-like growth factor-1(IGF-1)and insulin growth factor binding protein-3(IGFBP-3)in children with short stature.Methods:One hundred and forty children with dwarfism admitted to our hospital from March 2016 to March 2021 were selected and included in the Idiopathic short stature(ISS)group and Growth hormone deficiency(GHD)group according to their growth hormone(GH)levels after 2 drug provocation tests,and 40 healthy children with physical examination during the same period were selected and included in the control group.The bone mineral density and serum vitamin D,IGF-1 and IGFBP-3 levels of children in each group were detected,and the correlation between bone mineral density and serum indexes of children with short stature was calculated by Pearson correlation analysis.Results:The bone mineral density of children in the control group,ISS group and GHD group decreased in turn,and the difference between the groups was statistically significant(P<0.05).Children's serum 25-hydroxyvitamin D,IGF-1 and IGFBP-3 in the control group,ISS group and GHD group decreased in turn,and the difference between the groups was statistically significant(P<0.05).Pearson correlation analysis showed that the bone mineral density of ISS and GHD children was positively correlated with serum 25-hydroxyvitamin D,IGF-1 and IGFBP-3(r=0.627,0.660,0.625,P<0.05).Conclusion:Children with short stature caused by different causes have different degrees of decrease in bone density,and the decrease of bone density in children is closely related to the decrease of serum 25-hydroxy-vitamin D,IGF-1,IGFBP-3.
