摘要
目的 在腰椎滑脱症患者治疗中,分析椎弓根螺钉内固定+后外侧植骨融合术治疗的临床效果。方法方便选择2018年5月—2021年3月在新沂市人民医院接受治疗的56例腰椎滑脱症患者为研究对象,依据随机数表法分为对照组(28例,采取椎弓根螺钉内固定治疗)和观察组(28例,采取椎弓根螺钉内固定+后外侧植骨融合术联合治疗)。对比不同术式干预下两组疗效指标、疼痛程度及并发症发生率。结果 术后6个月观察组BFS评分(19.78±2.79)分、椎间隙高度(95.33±1.89)mm高于对照组(15.12±2.13)分、(90.59±1.48)mm,滑脱率及复位丢失率为(7.56±1.23)%、(7.75±0.62)%低于对照组的(12.30±1.85)%、(14.52±1.67)%,差异有统计学意义(t=7.025、10.448、11.290、20.110,P<0.001)。术后6个月疼痛程度比较,观察组评分(2.63±1.02)分较对照组(4.32±1.05)分更低,差异有统计学意义(t=6.109,P<0.001)。两组并发症发生率(7.14%vs 14.29%)对比,差异无统计学意义(χ^(2)=0.187,P>0.05)。观察组再次手术发生率0.00%,较对照组的21.43%显著降低,差异有统计学意义(χ^(2)=4.667,P<0.05)。结论 椎弓根螺钉内固定+后外侧植骨融合术在腰椎滑脱症治疗中效果显著,手术安全性高。
Objective In the treatment of patients with lumbar spondylolisthesis, the clinical effect of pedicle screw internal fixation + posterolateral bone graft fusion was analyzed. Methods Fifty-six patients with lumbar spondylolisthesis who were treated in Xinyi People’s Hospital from May 2018 to March 2021 were conveniently selected as the research objects, according to the random number table, were divided into the control group(28 cases, treated with pedicle screw internal fixation) and the observation group(28 cases, treated with pedicle screw internal fixation + posterolateral bone graft fusion combined treatment). The curative effect indexes, pain degree and complication rate were compared between the two groups under different surgical interventions. Results 6 moth after operation, the BFS score and the intervertebral space height of the observation group were(19.78±2.79) points(95.33±1.89) mm higher than those of the control group(15.12±2.13) points and(90.59±1.48) mm, the slippage rate and reduction loss rate were(7.56±1.23)%,(7.75±0.62)%, which were lower than(12.30±1.85)% and(14.52±1.67)% of the control group, the difference was statistically significant(t=7.025, 10.448, 11.290, 20.110, P<0.001). Comparing the degree of pain 6months after operation, the score of the observation group(2.63±1.02) points was significantly lower than that of the control group(4.32±1.05) points, the difference was statistically significant(t=6.109, P<0.001). The incidence of complications between the two groups was 7.14% vs 14.29%, and the difference was not statistically significant(χ^(2)=0.187,P>0.05). The incidence of reoperation in the observation group was 0.00%, which was significantly lower than that in the control group(21.43%), the difference was statistically significant(χ^(2)=4.667, P<0.05). Conclusion Pedicle screw internal fixation + posterolateral bone graft fusion has a significant effect in the treatment of lumbar spondylolisthesis,and the surgical safety is high.
