摘要
目的:观察苦参汤加减熏洗对混合痔外剥内扎术后创面愈合、疼痛及肛门功能的影响。方法:将86例混合痔患者按照随机数字表法分为对照组和观察组,每组各43例。对照组术后第2天给予高锰酸钾坐浴,观察组术后第2天给予苦参汤加减熏洗。比较两组患者的临床疗效、并发症发生情况、创面愈合时间、疼痛及治疗前后视觉模拟评分(visual analog score, VAS)、肛门功能变化情况。结果:对照组有效率为79.07%,观察组有效率为95.35%,两组有效率比较,差异具有统计学意义(P<0.05)。两组患者治疗后VAS评分低于本组治疗前,且治疗后观察组低于对照组,差异有统计学意义(P<0.05)。两组患者治疗后肛管高压带长度短于本组治疗前,且治疗后观察组短于对照组,两组患者治疗后肛管舒张压、肛管最大收缩压及肛管静息压高于治疗前,且治疗后观察组高于对照组,差异均有统计学意义(P<0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。观察组创面愈合时间短于对照组,差异有统计学意义(P<0.05)。结论:苦参汤加减熏洗可减轻混合痔外剥内扎术后疼痛程度,改善肛门功能,减少并发症的发生,促进创面愈合。
Objective: To observe the influence of Modified Radix Sophorae Flavescentis Decoction for fumigation and washing on wound healing and pain and anal function after Milligan-morgan Hemorrhoidectomy(MMH) of Mixed Hemorrhoids(MH).Methods: A total of 86 patients with MH were divided into the control group and the observation group according to the method of random number table, with 43 cases in each group.The control group was given potassium permanganate for sitz bath on the second day after operation, while the observation group was given Modified Radix Sophorae Flavescentis Decoction for fumigation and washing on the second day after operation.The clinical efficacy, complications, wound healing time, pain, the changes of visual analog score(VAS) and anal function of the two groups before and after treatment were compared.Results: The effective rate was 79.07% in the control group and 95.35% in the observation group.The difference between the two groups was statistically significant(P<0.05).The VAS score of the two groups after treatment was lower than that of the same group before treatment, and the VAS score of the observation group after treatment was lower than that of the control group.The difference between the two groups was statistically significant(P<0.05).After treatment, the length of anal high pressure zone in the two groups was lower than that before treatment, and the index in the observation group was lower than that in the control group.The anal diastolic pressure, anal maximum systolic pressure and anal resting pressure were higher than those before treatment, and all the differences were statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group.The difference between the two groups was statistically significant(P<0.05).The wound healing time of the observation group was shorter than that of the control group.The difference between the two groups was statistically significant(P<0.05).Conclusion: Modified Radix Sophorae Flavesce
