宫颈癌患者放疗体位固定辅助装置优选及其对摆位精度的影响 被引量:4

Optimal selection of postural fixation aids for radiotherapy patients with cervical cancer and their impacts on setup accuracy

摘要 目的:探究体位固定辅助装置对宫颈癌放疗摆位精度的影响,并提出体位固定辅助装置的优选方案。方法:回顾性选取某院57例宫颈癌放疗患者,根据体位固定辅助装置的不同分为A组(20例)、B组(16例)和C组(21例),3组患者头下均垫C型头枕,其中B、C组头枕与体板之间相对位置固定,A、B组患者膝下加硬质海绵膝垫。比较3组间左右(x轴向)、升降(y轴向)、进出(z轴向)、旋转(Rtn)、进出倾斜(Pitch)和左右转动(Roll)6个维度的摆位误差、误差分布和异常摆位次数。采用SPSS 25.0软件进行统计学分析。结果:C组摆位误差除Roll方向外均优于A、B 2组,且Pitch方向差异有统计学意义(P<0.001)。C组误差分布除Roll方向外均优于A、B 2组,且Pitch方向差异有统计学意义(χ^(2)=17.603,P<0.001)。相较A、B 2组,C组异常摆位次数大幅降低,差异有统计学意义(χ^(2)=7.307,P=0.026)。结论:宫颈癌放疗时将头枕与体板之间相对位置固定并去除患者膝下的硬质海绵膝垫,有助于提高摆位精度、降低重复摆位次数、提高治疗效率。 Objective To investigate the effects of postural fixation aids on radiotherapy setup accuracy for cervical cancer patients so as to propose an optimal selection solution of postural fixation aids. Methods Totally 57 radiotherapy patients with cervical cancer were selected retrospectively from some hospital and divided into A(20 cases), B(16 cases) and C(21 cases)groups according to the differences between the postural fixation aids. The C-shaped head pillows were placed under the heads of the three groups of patients, with the relative positions between the pillows and body plates fixed in groups B and C and the rigid sponge knee pads under the knees of patients in A and B groups. The setup errors, error distributions and the number of abnormal setup errors were compared among the three groups in six directions( X-, Y-, Z-axis, Rtn, Pitch and Roll). SPSS 25.0 software was used for statistical analysis. Results C group gained advantages over the other two groups in setup errors in all the directions except for Roll direction, and the difference in Pitch direction was statistically significant(P<0.001). The error distribution in group C was better than those in groups A and B in all the directions except for Roll direction, and the difference in Pitch direction was statistically significant( χ^(2)=17.603, P<0.001). C group had the number of abnormal setup errors significantly lower than those of the other two groups( χ^(2)=7.307, P=0.026). Conclusion Fixing the relative position between the headrest and the body plate and removing the rigid sponge knee pad under the patient’s knee during radiotherapy for cervical cancer help improve setup accuracy, reduce the number of repeated setup and enhance treatment efficiency.
